Abstract

AIM: In this study, we compared the different opioids regarding hemodynamic stability, postoperative recovery and impacts on side effects in laparoscopic surgery. MATERIAL AND METHODS: ASA I and II 32 patients who were candidates of elective laparoscopic cholecystectomy were included. All patients received 25 mg meperidine and 0.5 mg atropin IM 1.5 hours before the operations. ECG, noninvasive blood pressure, peripheral oxygen saturation (SpO2) and CO2 in expiration air (ETCO2) were monitorized in the operating room. Group AS patients had IV induction with 10 µgr/kg alfentanil, 2 mg/kg propofol and 0.6 mg/kg neuromuscular blocking rocuronium. Group FS was given 2 µgr/kg fentanyl 2 mg/kg propofol and 0.6 mg/kg rocuronium for the same purpose. After controlled ventilation with face mask for 2 minutes and tracheal intubation, anesthesia maintanance was established with 0.05 µgr/kg/hour alfentanil infusion and 2% sevoflurane in 50% O2 and N2O mixture in Group AS and 0.025 mgr/kg/hour fentanyl infusion and the same doses of sevoflurane in Group FS. Intraoperative and postoperative hemodynamics and postoperative recovery patterns were recorded. RESULTS: Groups were similar as to demographic data. Intraoperative and postoperative heart rate and blood pressure changes and SpO2 and ETCO2 recordings throughout the study were found to be statistically similar. CONCLUSION: In this study, we reached the conclusion that intravenous opioids in combination with inhalation anesthetics provide balanced anesthesia and have negligible side effects in laparoscopic surgery.

Özet

AMAÇ: Bu çalışma ile, opioidlerin laparoskopik cerrahide indüksiyon ve anestezi idamesinde hemodinamik açıdan stabilitelerini, postoperatif derlenme karakterlerini ve yan etkilere olası etkilerini karşılaştırılması amaçlanmıştır. GEREÇ VE YÖNTEM: Hastanemizde elektif laparoskopik kolesistektomi uygulanacak ASA I-II grubu 32 hasta çalışmamıza dahil edildi. Çalışmaya dahil edilen hastalara operasyondan 1,5 saat önce 25 mg dolantin ve 0,5 mg atropin IM olarak uygulandı. Takiben hastalar ameliyathaneye alındığında, EKG, noninvazif kan basıncı, periferik hemoglobin oksijen doygunluğu (SpO2), ekspiryum havasındaki karbondioksit miktarı (ETCO2) monitorizasyonu yapıldı. Grup I'e (Grup AS) indüksiyonda 10 µgr kg-1 alfentanil, 2 mg kg-1 propofol, nöromüsküler blokaj için 0,6 mg kg-1 roküronyum IV olarak verildi. Grup II'ye (Grup FS) indüksiyonda 2 µgr kg-1 fentanil, 2 mg kg-1 propofol, nöromüsküler blokaj için 0,6 mg kg-1 roküronyum verildi. İki dakika yüz maskesi ile kontrollü ventilasyonu takiben, hastalarda endotrakeal entübasyon gerçekleştirildi. Anestezi idamesinde Grup AS'ye 0,05 µgr kg-1 saat-1 alfentanil infüzyonu, %50 O2 %50 N2O karışımı içinde %2 sevofluran, Grup FS'ye 0,025 µgr kg-1 saat-1 fentanil infüzyonu, %50 O2 %50 N2O ka rışımı içinde %2 sevofluran verildi. İntraoperatif ve postoperatif hemodinamik takipler ve postoperatif derlenme karakterleri incelendi. BULGULAR: Demografik data açısından gruplar arasında fark bulunamamıştır. İntraoperatif ve postoperatif kalp atım sayısı ve kan basıncı ölçümleri her iki grupta benzer bulunmuştur. SpO2 ve ETCO2 değerleri yine gruplar arasında benzerdir. SONUÇ: Bu çalışmamızın sonucunda inhalasyon anesteziklerine ek olarak kullanılan opioidlerin dengeli anestezi sağladığı ve postoperatif yan etkilerin kabul edilebilir seviyede olduğunu tespit ettik.
