Abstract

Fasting and refeeding have considerable effects on thyroid hormone metabolism. A study comprising 23 males (mean ages; 30.38 ± 6.74 years), 21 females (mean ages;33.26 ± 7.60 years) was conducted during ramadan to compare free T3 (FT3), free T4 (FT4), TSH levels in fasting with the levels of nonfasting conditions. Each individual gave two blood samples: First sample was taken 2 days before the onset of Ramadan, the second sample was taken at the end of Ramadan. 31 of the group ate twice including a meal at dawn time while 13 ate only once at sunset during Ramadan period body mass index (BMI), FT3, FT4, TSH were measured of the hall group before and after Ramadan period. There was an increase in TSH levels (within normal limit) and decrease in BMI in hall group. While there was not any difference in pre and post Ramadan levels in woman, a statistically significant decrease in BMI and increase in TSH (within normal limit) were established in men. There was no difference in the group who had two meals a day ( including dawn time) . On the other side significant decrease in BMI and increase in TSH were established in the group who ate only once a day during the Ramadan period. These changes showed the importance of the length of fasting period in effecting thyroid function. This seguence of events is consistent with the concept that pituitary thyrotrophs have gained increased negative feedback sensitivity to circulating T3 in fasting.

Özet

Açlık ve beslenmenin tiroid hormon metabolizması üzerine önemli etkileri vardır. Bu çalışmaya 23 'ü erkek (yaş ortalaması 33.26±7.6 yıl), 21 'i kadın (yaş ortalaması 30.38 ±6.7 yıl) toplam 44 sağlıklı kişi alınarak Ramazan ayında açlık ile serbest T3, serbest T4, TSH değişimi araştırıldı. Çalışmaya alınan gruptan 2 kan örneği alındı: Birinci kan örneği Ramazan ayından 2 gün önce, 2. kan örneği Ramazan ayının bitiminde alındı. 31 'i sahura kalktı, 13 kişi ise sahur yapmadan oruç tuttu. Çalışmaya alınan tüm grubun Ramazan öncesi ve sonrası beden kitle indeksi (BKI) , serbest T3 (ST3), serbest T4 (ST4) , TSH düzeyleri incelendi. Ramazan öncesi ve sonrası tüm grubun verileri karşılaştırıldığında BKI'inde anlamlı azalma (p<0.05), TSH 'da (normal limitlerde kalmasına karşın) ise anlamlı artış (p<0.05) tespit edildi. Kadınlar tek başına ele alındığında Ramazan öncesi ve sonrası değerlerde anlamlı fark saptanmaz iken, erkeklerde BKI'inde anlamlı azalma (p<0.05), TSH'da ise anlamlı artış (p<0.05) tespit edildi. Sahura kalkan grupta Ramazan öncesi ve sonrası parametrelerde anlamlı değişiklik saptanmadı. Sahura kalkmayan grupta BKI'inde anlamlı azalma (p<0.05), TSH'da ise anlamlı artış (p<0.05) tespit edildi. TSH'daki artış ve BKI'indeki azalmanın sahura kalkmayan grupta tespit edilmesi, tiroid hormon fonksiyonlarındaki değişikliğin daha ziyade açlığın daha uzun olduğu grupta ön plana çıktığını göstermiştir. Açlıkta ST3, ST4 düzeylerinde değişiklik olmaksızın TSH'daki artışın nedeni; hipofizer tirotrofların açlıkta azalma eğilimi gösteren T3 'ü normal sınırlarda tutmak için TSH salınımını artırması ile izah edilebilir.
