Abstract

Foot infection is a frequently encountered condition caused by neuropathy, ischemia and arthropahty in patients with diabetes mellitus. 124 patients -with diabetic foot infection who are hospitalized and treated from 1995 to 1998 in Endocrinology clinic are evaluated retrospectively. Of these 13 (%10.5) were type 1 DM (age 24.3±4.4), 111 (%89.5) were type 2 DM (age 54.0$\pm$7.2). Ten (8.1%) of the patients were on diet therapy, 46 patients (37.1%) on oral antidiabetics, and 68 (54.8%) on insulin. Mean duration of diabetes was 8.4±5.0 years. 69 (55.6%) had neuropathy, 65 (52.4%) had nephropathy, and 87 (70.2%) had retinopathy. Diabetic foot infections were grade 1 in 6 patients (4.8%), grade 2 in 74 (59.7%), grade 3 in 14 (11.3), and grade 4 in 30 (24.2%) patients. 41 of the patients (33.1%) had osteomyelitis. Foot infections healed with medical treatment in 89 patients, and amputation has been performed in 35 (28.2%). In conclusion our data shows that diabetic foot infection is still a common and serious problem

79'u erkek, 45'i kadın olan hastaların 13'ünde (%10.5), tip 1 diabet, 111'inde (%89.5) Tip 2 diabet vardı. Hastaların yaş ortalamaları Tip 1 diabetik hastalarda 24.3±4.4, Tip 2 diabetik hastalarda ise 54.01±7.2 idi. Hastaların 10'u (%8.1) diyet, 46'sı (%37.1) oral antidiabetik, 68'i (%54.8) insulin kullanmaktaydı. Diabet süreleri 8.4±5.0 yıl olan hastaların %55.6'sında (69 hastada) diabetik nöropati, %52.4'ünde (65 hastada) diabetik nefropati, %70.2'sinde (87 hasta) diabetik retinopati tespit edildi. Wagner sınıflamasına göre hastalarınn diabetik ayak grade'leri 6 hasta (%4.8) grade 1, 74 hasta (%59.7) grade 2, 14 hasta (%11.3) grade 3 ve 30 hasta (%24,2) grade 4 idi. Hastaların %33.1'inde (41 hasta) osteomiyelit tespit edildi. Hastaların %71.8'i (89 hasta) medikal tedavi ile iyileşirken %28.2'sine (35 hasta) amputasyon uygulandı. Bu bulgular diabetik ayak infeksiyonunun diabetli hasta popülasyonunda hala yaygın ve ciddi bir problem olduğunu ortaya koymaktadır.
