Abstract

OBJECTIVE: To compare the efficacy and side effects of two different combinations of local anesthetic and hipnotic drug in epidural anesthesia with the systemic application of meperidine for spontaneous nulliparous labor. MATERIALS AND METHODS: Thirty nulliparous women in term labor with cervical dilatation of 3-4 cm were randomly divided into two groups. After placement of epidural catheter, 1. group were given 10 ml bupivacaine of 0,125 % and 2. group were given 10 ml bupivacaine of 0,125% plus 50 mgr fentanyl. 3. Group (n=15) were injected repeated doses of 50 mg meperidine intramuscularly for systemic analgesia as control. RESULTS: Duration of labor was not significantly different among three groups (200±71,6 min. and 219±71,3 min. and 230,5+72,5 min.) (p=0,43). Time interval for second dose of analgesia and need for oxytocine and apgar scores were not significantly different either. The analgegic efficacy was significanly higher in regional anesthesia group (p<0.0001). Postpartum headache in regional anesthesia group and nausia and vomiting in systemic anesthesia group were higher and other side effects and complications similar among groups. CONCLUSION: Epidural anesthesia is very effective for labor analgesia and is the method of choise. Ten ml bupivacaine of 0,125% plus 50 mgr fentanyl is found effective and safe dose with minimal side effects if performed by experienced staff.
