Abstract

AIM: Some epidemiological studies have reported that high iron stored level is a risk factor for coronary artery disease. It has been supposed that increases in serum lipids might be one of the iron-induced atherogenic mechanisms. However, iron-deficient diets have caused contradictory lipids changes in rats. The aim of this study was to investigate the changes of serum lipids after treatment of iron deficiency anemia in premenopausal women. MATERIAL AND METHODS: Thirty premenopausal women (mean age 38.9±2 years) with iron deficiency anemia were included in this study. Hypertension, hypothyroidism, being on treatment of hyperlipidemia, iron deficiency anemia or estrogen containing drugs, body mass index over 27 and acute blood loss were the exclusion criteria. RESULTS: After hemoglobin restoration, there were significant increases in total cholesterol (mg/dl) (from 182.3±6.9 to 191.7±7.5), low density lipoprotein cholesterol (mg/dl) (from 114±6.1 to 121±6.2), apolipoprotein A (mg/dl) (from 138.1±5.4 to 145.1±5.9) and apolipoprotein B (mg/dl) (from 99.4±6.6 to 105±7.5) (p=0.006, p=0.031, p=0.048 and p=0.03, respectively). However, there were no significant changes in high density lipoprotein cholesterol, triglyceride and glucose levels (p=0.119, p=0.92 and p=0.418, respectively). CONCLUSION: Our results give support to the possible association of iron with serum lipids increase.

Özet

AMAÇ: Epidemiyolojik çalışmalar fazla miktardaki depo demirinin koroner arter hastalığı açısından önemli risk faktörü olduğunu bildirmişlerdir. Serum lipidlerindeki artışın demirin aterojenik mekanizmalarından birisi olabileceği ileri sürülmüştür. Ancak sıçanlarda demirden fakir diyetlerin serum lipidlerine olan etkisini araştırmak amacıyla yapılan çalışmalarda çelişkili sonuçlar alınmıştır. Bu çalışmanın amacı menopoz öncesi kadınlarda demir eksikliği anemisi tedavisinin serum lipidlerinde yol açabileceği değişiklikleri araştırmaktır. GEREÇ VE YÖNTEM: Bu çalışmaya 30 demir eksikliği anemisi olan premenopozal kadın (ortalama yaş 38.9±2 yıl) alındı. Çalışmaya alınmama kriterleri hipertansiyon, hipotiroidi, hiperlipidemi veya demir tedavisi altında olmak, östrojen preperatı kullanıyor olmak, vücut kitle indeksinin 27'den büyük olması veya akut kan kaybının olması idi. BULGULAR: Hemoglobin düzeyleri düzelince total kolesterol (mg/dl) (182.3±6.9'dan 191.7±7.5'e), düşük dansiteli lipoprotein kolesterolü (mg/dl) (114±6.1'den 121±6.2'ye), apolipoprotein A (mg/dl) (138.1±5.4'ten 145.1±5.9'a) ve apolipoprotein B (mg/dl) (99.4±6.6'dan 105±7.5'e) seviyelerinde anlamlı artışlar saptandı (sırasıyla p=0.006, p=0.031, p=0.048 ve p=0.03).
