Abstract

After brain death, organ harvesting should be performed as soon as possible, because of the presence of hemodynamic instability. During the evaluation of the donor, the application of all conventional diagnostic procedures including the detection of the infections confront difficulties. Infections in immunosuppressive organ recipients occurring after post-transplant period are one of the important life threatening risk factors. Detection of the preexisting infections in the donor and initiation of the appropriate antimicrobial treatment may prevent the transmission of donor infections to the recipients. Despite all the precautions taken, some donor derived infections, may be transmitted to the recipient. The incidence of bacteraemia after transplantation changes between 8.6-26 % depending on the type of the organ. And the bacteraemia is seen as the most important factor for mortality and morbidity. This condition has importance especially for the multi-drug resistant bacteria. According to American Infectious Disease Committee, these group microorganisms are grouped as ESKAPE pathogens (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa and Enterobacter spp.). In this group, Pseudomonas infections are seen as the most fatal pathogens and almost 50% of the pseudomonas infections detected in the organ recipients are multi-drug resistant. Previously, Pseudomonas infections are reported in solid organ transplantation, but donor derived pseudomonas infections are very rare. It is showed that Pseudomonas infections may cause pseudoaneurysm and disruption in the arte- rial anastomosis in early post-transplant period. This condition may result with graft loss or patient death. Elastase enzyme produced by the bacteria cause the endothelial cell and basal membrane damage and bleeding from the vessel wall. In this case report, complications and mortality in a kidney recipient transmitted from deceased donor due to Pseudomonas infection with suspicion of donor derived will be presented.
