Abstract

We presented the case report of a 63 year-old female suffering from destructive rheumatoid poliarthritis(RA). She had complaints of progressive gait impairment, dysphagia, dyspne, and numbness in hands and feet. Neurological examination showed an unstable gait and pyramidal tract signs. Anterior occipitoatlantoaxial subluxation with pannus formation cranial settling and cervical myelopathy were demonstrated using conventional X-ray studies and MRI. She was treated with a traction in the bed. She had performed an occipito servikal Y-plac stabilization and fusion operation. She had no complaints after postoperative 2 day and neurological examination showed an stable. The reported rate of neurological impairment due to cranial settling and luxation ranges from 7 to 20%Radiographic analysis indicates that the posterior atlantoodontoid interval (or <= 14 mm) is an important parameter.
