Abstract

25 years old male patient attempted to our clinic with a complaint of vision decrased in the left eye on 30.06.2005. In his history, his left eye had been injured with fish hook at 07.06.2005. Then corneal primer suturation had been done , local goverment hospital, in his left eye. On eye examination; in his right eye was normal and visual acuity was 10/10. Eye external structure and eye movement were normal in his left eye. Corneal full thickness wound, which had been closed with 10/0 nylon suture, on 8 o’clock of cornea, traumatic anridia(almost totally), swollen lens, traumatic cataract within the left eye in the slitlamp examination. Left eye’s retina could not be visualized. According to the ocular ultrasonograhy; retinal detachment and intraocular hemorrhage were seen in left eye. Left eye visual acuity was P+/P+. After that lens aspiration, pars plana vitrectomy, endolaser fotocoagulation and silicon oil were enjected in first step. After 6 month, his left eye’s retina was normal, silicon oil remove and aniridia IOL was implanted for aphakia and aniridi. Artificial aniridia IOL useful for aphakia and traumatic aniridi repairment.

Özet

Yirmibeş yaşında erkek hasta sol gözünde görmesinin azaldığını ifade ederek 30.06.2005 tarihinde kliniğimize başvurdu. Hikayesinde 07/06/2005 tarihinde sol gözüne olta battığını ve aynı gün en yakınındaki devlet hastanesinde korneal primer sütürasyon yapıldığını ifade etmekteydi. Göz muayenesinde; sağ göz sağlam ve görmesi tamdı. Sol göz external yapıları tabii olup göz hareketleri her yöne serbestti. Biyomikroskopik muayenede saat 8 hizasında primer tamir yapılmış skleradan kornea santraline uzanan tam kat korneal kesi, totale yakın aniridi, lens şişkin ve travmatik katarakt mevcuttu. Fundus muayenesinde retina seçilememekle beraber oküler ultrasonografide nasal kadran retina dekolmanı ile uyumlu ultrasonografik görüntü alındı.Görmesi P+/P+’di. Hastaya ilk seansda lens aspirasyonu, pars plana vitrektomi endolaser fotokoagülasyon ve silikon yağı enjeksiyonu uygulandı. Altı ay sonra retina yatışık olarak izlendikten sonra silikon yağı boşaltımını takiben aynı seansta hastaya aniridi göz içi lens (GİL) implantasyonu uygulandı. Suni iris-lens diyaframlı aniridi GİL’leri afaki ve travmatik aniridinin birlikte olduğu olguların düzeltilmesinde yararlıdır.
