Abstract

Investigation of the Activity of Fosfomycin and Nitrofurantoin to Extended Spectrum Beta-lactamase Producing Escherichia coli and Klebsiella pneumoniae Strains Isolated from Hospitalized Patients with Urinary Tract InfectionsThe isolation of extended-spectrum beta-lactamase (ESBL) producing Escherichia coli and Klebsiella pneumoniae in urinary tract infections constitutes an important therapeutic challenge. In this study in vitro susceptibility of fosfomycin and nitrofurantoin against ESBL-producing isolates consisting of 71 Escherichia coli and 27 Klebsiella pneumoniae isolated in Ataturk University Hospital Microbiology Laboratory between June 2014 and January 2016 from urine was determined. We also compared susceptibility of other antibiotics that can be used to treat UTIs caused by ESBL producing isolates, to fosfomycin and nitrofurantoin. ESBL activity and susceptibilities to antibiotics were determined by Kirby-Bauer disk diffusion method according to CLSI standards. Fosfomycin susceptibility of K.pneumoniae strains was determined taking in to account breakpoint for E.coli.The susceptibility ratios of fosfomycin and nitrofurantoin for ESBL producing E.coli was 90.1 %; 91.5 % and for K.pneumoniae 83.7 %, 85.7 % respectively. For all isolates the susceptibility ratios of ampicillin-sulbactam, trimethoprim-sulfamethoxazole and ciprofloxacin were 18.4 %, 25.5 % and 35.7 % respectively. The efficacy of fosfomycin and nitrofurantoin was significantly higher when compared with ampicillin-sulbactam, trimethoprim-sulfamethoxazole and ciprofloxacin (p<0.01) In conclusion, as their high in vitro activitiy fosfomycin and nitrofurantoin both can be an important alternative for treatment of UTIs caused by ESBL producing E.coli and K.pneumoniae
