Abstract

Aim: To evaluate retrospectively the patients treated in our department with a diagnosis of diabetic ketoacidosis. Methods: The files of the patients were evaluated regarding gender, age, symptoms, clinical features, diabetic stories, the pre-treatment and laboratory findings. Findings: The mean age 34.93 ± 17.5 to 6 females and 8 males were enrolled in the study. The most common symptoms of nausea, vomiting, respectively and confusion. Insufficient dose of insulin usage in five cases and sepsis due to urinary infection in 7 cases were the reason of ketoacidosis. Only one case requested emergency hemodialysis among 7 cases with high level of urea/creatinin. Mean 40.5 ± 6 hours in patients underwent insulin infusion. They stayed an average of 4.43 ± 2.6 days in the hospital. There was no complication or mortality. Result: Patients with DKA, requiring an accurate and rapid diagnosis, should be managed appropriate therapy. We observed that DKA in adult age group can be successfully treated by an energetically approach.

Özet

Amaç: Bu çalışmada diyabetik ketoasidoz (DKA) tanısı ile hastanemiz Dahiliye Kliniğine son bir yılda yatırılan hastaları değerlendirmek. Yöntemler: Hastaların dosyaları geriye dönük incelendi. Olguların cinsiyet, yaş, yakınma, klinik özellikleri, diyabet öyküleri, yatış öncesi uygulanan tedavi ve laboratuvar bulguları incelendi. Bulgular: Ortalama yaşları 34,93 ±17,5 olan 6 kadın, 8 erkek hasta çalışmaya alındı. En sık yakınmalar sırasıyla bulantı -kusma, bilinç bulanıklığı idi. hastalarda takipne ik ve asidotikti.5 olguda yetersiz insülin kullanımı ve 9 olguda idrar yolu infeksiyonu saptandı. Üre/kreatinin düzeyi yüksek 7 olgudan yalnızca birinde acil hemodiyaliz gerekti. Hastalara ortalama 40,5 ± 6 saat insülin infüzyonu uygulandı. Ortalama 4,43±2,6 gün hastanede yoğun insülin tedavileri uygulandı. Komplikasyon veya ölüm görülmedi. Sonuç: DKA'lu hastalar, doğru ve hızlı tanı konarak komplikasyonlar göz önünde bulundurulup uygun tedavi edilmelidir. Erişkin yaş grubundaki DKA tanısı ile izlediğimiz olgularda enerjik tedaviyle başarılı sonuçlar alınacağını gördük.
