Abstract

Objective: Investigation of the characteristics of bowel care and determining the effect of severity of injury, neurologic level and ambulation on characteristics of bowel care in patients with spinal cord injury (SCI). Methods: 55 patients with SCI were taken into our study. Severity of the injury, neurologic level (upper and lower motor neuron neurogenic bowel dysfunction, UNMD and LMND), ambulation status, place of bowel care, rate of need for support, weekly frequency of defecation and duration of bowel care (less than 15 minutes, 15-30 minutes, 30-60 minutes) were studied. Results: 31 (56.4%) patients had complete, and 24 (43.6%) patients had incomplete injury. 44 (80%) patients had UNMD, 11 (20%) patients had LMND. 15 (27.3%) patients were ambulatory while 40 (72.7%) were non- ambulatory. No significant difference was found between weekly median frequencies of defecation by severity of the injury (p=0.609). However, weekly median frequency of defecation in LMND and ambulatory patients were found to be significantly higher than that in UMND and non-ambulatory patients (respectively p=0.034, p=0.034). There was neither found any significant difference between durations of bowel care by severity of injury (p=0.169), while it was found significant difference between durations of bowel care by neurological level (p=0,025). It was also found that 80% of the ambulatory patients completed bowel care in less than 15 minutes. Conclusion: Ambulation increases frequency of defecation and shortens the duration of bowel discharge. Therefore, mobilizing the patients as early as possible is important.

Özet

Amaç: Spinal kord yaralanmalı (SKY) hastalarda bağırsak bakımı özelliklerinin incelenmesi ve bağırsak bakımı özelliklerine, hastaların yaralanma şiddeti, nörolojik seviye ve ambulasyon durumunun etkisinin saptanmasıdır. Yöntemler: Çalışmamıza 55 SKY'li hasta alındı. Hastaların yaralanma şiddeti, nörolojik seviye (üst ve alt motor nöron nörojenik barsak disfonksiyonu, ÜMND ve AMND) ambulasyon durumu, bağırsak bakımını gerçekleştirdikleri yer, destek ihtiyaç oranları, haftalık defekasyon sıklıkları ve bağırsak bakım süreleri (15 dakika altı, 15-30 dakika, 30-60 dakika) incelendi. Bulgular: Otuzbir (% 56,4) hasta komplet, 24 (% 43,6) hasta ise inkomplet yaralanmaya sahipti. Hastaların 44'ünde (%80) ÜMND, 11'inde (%20) ise AMND mevcuttu. Hastaların 15'i (%27,3) ambule iken, 40 hasta (%72,7) ambule değildi.
