Abstract

Secretions, hyperreactive airways, tracheal stimulation, and anesthetic agents may lead to bronchospasm, inadequate ventilation, and even cardiac arrest (1). In one study, bronchospasm was diagnosed in 1.7 per 1,000 anesthetic procedures (2). In such cases, ventilator dyssynchrony, high airway pressures, prolonged expiration, and loss of tidal volume may be observed (3). Early recognition, initiation of bronchodilator and corticosteroid (prednisolone) therapy, and the implementation of appropriate ventilatory strategies are of vital importance (4). Administering a beta-2 agonist 30 minutes before surgery, using propofol for induction, and ensuring adequate anesthetic depth before airway manipulation reduce the risk of bronchospasm (5). In this case report, we present the successful resuscitation of a patient with a hyperreactive airway who developed arrest secondary to bronchospasm following induction.

Özet

Sekresyonlar, hiperaktif havayolu, trakeal uyarı ve anestezik ilaçlar bronkospazma ve ventilasyon yetersizliğine, hatta arreste sebep olabilir (1). Bir çalışmada 1000 anestezide 1,7 oranında bronkospazm tanısı konulmuştur (2). Bu durumda ventilatör uyumsuzluğu, yüksek havayolu basıncı, uzamış ekspirasyon ve tidal hacim kaybı gözlenebilir (3). Erken tanı, bronkodilatör ve prednol tedavisinin başlanması ile uygun ventilatör stratejilerinin uygulanması hayati önem taşır (4). Ameliyattan 30 dakika önce beta-2 agonist uygulanması, indüksiyon için propofol kullanılması ve havayolu manipülasyonundan önce yeterli derinlikte anestezi sağlanması bronkospazm riskini azaltır (5). Bu olguda indüksiyon sonrası hiperaktif havayolu bulunan hastada gelişen bronkospazma bağlı arrest üzerine uyguladığımız başarılı resüsitasyon sunulmuştur.
