Abstract

Acute respiratory failure developing after “near-drowning’’ can progress rapidly as a result of aspiration, inflammation, and surfactant inactivation. During this process, macrodynamic hemodynamic parameters may appear stable; however, ongoing microcirculatory hypoperfusion can lead to tissue hypoxia and persistent hyperlactatemia. Elevated lactate levels may reflect not only hypoperfusion but also metabolic remodeling. A 76-year-old female patient was admitted to the intensive care unit after being intubated for acute respiratory failure following “near-drowning’’. Despite mechanical ventilation and balanced fluid resuscitation, progressive hyperlactatemia developed. Although the patient remained hemodynamically stable and did not require inotropic support, monitoring with the PRAM-based MostCare system, which analyzes the arterial pressure waveform, revealed that cardiac index, systemic vascular resistance index, pulse pressure variation, and stroke volume variation were within normal ranges. Nevertheless, the continued rise in lactate levels suggested persistent microvascular hypoperfusion. An isoprenaline infusion initiated at the 16th hour of ICU admission was followed by a rapid and progressive decline in lactate levels. In parallel with clinical and metabolic improvement, the patient was successfully extubated and transferred to the ward. This case demonstrates that macrodynamic hemodynamic stability does not always reflect adequate microcirculatory perfusion in acute respiratory failure following “near-drowning’’. Persistent hyperlactatemia may serve as an important indicator of microcirculatory dysfunction. The rapid lactate clearance observed with isoprenaline therapy suggests that β-adrenergic agents may have the potential to improve microcirculation and oxygen delivery in selected patients. In critically ill patients, evaluating lactate as a trend rather than a single value and individualizing treatment strategies according to circulatory phenotype are critical.
