Abstract

Aim: This study aimed to evaluate the clinical, radiological, surgical, and histopathological characteristics of patients with spinal intradural tumors who underwent microsurgical resection. In addition to describing tumor distribution and surgical outcomes, the study sought to identify technique-related trends that may guide treatment strategies. Materials and Methods: A retrospective analysis was performed on 19 patients who underwent surgery for spinal intradural tumors between 2015 and 2025. Demographic data, presenting symptoms, ASIA scores, tumor location, histopathology, surgical technique, extent of resection, and postoperative complications were recorded. All procedures were conducted using microsurgical techniques under fluoroscopic control and continuous intraoperative neuromonitoring (SSEP/MEP). Because of the small sample size and absence of comparison groups, only descriptive statistics were used. Results: Pain was the most common presenting symptom (89.5%), followed by sensory disturbances and gait impairment. Schwannoma was the most frequent tumor type (57.9%), and the thoracic region was the most commonly affected level. Total resection was achieved in 84.2% of patients. Laminectomy with stabilization was the predominant surgical approach, while laminoplasty and hemilaminectomy were used for selected cases. Postoperative complication rate was low (10.5%), including one cerebrospinal fluid fistula and one case of transient paraplegia. No postoperative deformity was observed during follow-up. Conclusion: Spinal intradural tumors, although uncommon, can mimic more frequent spinal disorders, leading to diagnostic delays. Microsurgical resection supported by neuromonitoring provides high rates of total excision with favorable functional outcomes. The findings of this single-surgeon, 10-year series contribute additional insight into technique selection and postoperative safety in the management of intradural tumors.
