Abstract

Thrombotic thrombocytopenic purpura (TTP) is lifethreatening disease characterized by microangiopatic hemolytic anemia (MAHA) and thrombocytopenia; commonly associated with malignancy, drugs and autoimmune disease. There may not always together microangiopatic hemolytic anemia, thrombocytopenia, neurological findings, renal dysfunction and fever symptoms in patient with TTP. Therefore, thrombocytopenia and microangiopathic hemolytic anemia should suggest the presence of TTP. Emergency admitted 65 years old woman with signs of confusion and agitation; laboratory findings is indirect bilirubin elevation, thrombocytopenia, anemia and LDH elevation, for further evaluation and treatment were admitted to intensive care. Patients with low levels of the enzyme ADAMTS13 been identified as having been diagnosed with TTP. Plasmapheresis and corticosteroid treatment was started to the patient. Raynaud’s phenomenon is reported by the patient in winter, and elevations in liver function tests when association with autoimmun disease was made further investigation. Antinuclear antibodies were detected patients received the diagnosis of 'I'I'P associated with SLE. Treatment with quinine and plaquenil enters diseased is remission and the patient was discharged.
