Abstract

Dear Editor, I read with great interest the article titled “Clinical and Epidemiological Characteristics of Interhospital Transfers to a Tertiary Care Center,” published in the recent issue of your journal (1). This study by Kahveci and Arı makes a significant contribution to the literature by revealing regional transfer processes and highlighting the clinical and organizational aspects of interhospital transfers. The purpose of this letter is to provide methodological clarification and constructive commentary that may assist in the interpretation of the study’s findings. I would like to draw attention to two points that may influence the interpretation of the statistical results. First, the changes in the number of transfers and the rates of intra-city and inter-city transfers over the years were reported to be statistically significant (e.g., p = 0.001 for differences between years in Table 1). For this analysis, the Mann–Whitney U test was used. While this test is appropriate for certain pairwise comparisons involving continuous or ordinal variables, alternative statistical approaches might also be considered when comparing categorical data or rates across multiple years (2021–2025). In such contexts, methods such as chi-square tests or logistic regression models may help more clearly address comparisons across multiple groups defined by year (2,3). Secondly, although the data for 2025 covers only a five-month period, it is presented in the same table and included in the same statistical comparisons as the full-year data from 2021–2024. This difference in observation duration, which may already be apparent to readers, could influence the interpretation of trends over time, as the relatively lower number of transfers in 2025 is likely related to the shorter time frame. Presenting the 2025 data separately or standardizing comparisons by duration might be considered as an approach to enhance the interpretability of the findings (4,5). I believe that clarifying these methodological points will further strengthen the interpretability of the findings.

Özet

Sayın Editör, “Clinical and Epidemiological Characteristics of Interhospital Transfers to a Tertiary Care Center” başlıklı ve derginizin son sayısında yayımlanan makaleyi büyük bir ilgiyle okudum.
