Abstract

Aim: In this study, it is aimed to calculate the current median values of prenatal screening tests performed in our laboratory and to compare them with the median values in the package program we use in risk calculation. Material and Methods: As a result of the applications made to our hospital from İzmir province and its surroundings between May 2019 and May 2020, 3264 first trimester screening tests and 954 second trimester screening tests were retrospectively analyzed. All parameters were studied from maternal serum in the Medical Biochemistry Laboratory of Izmir Katip Çelebi University Atatürk Training and Research Hospital. The median values and median multiples of the data obtained according to gestational weeks were calculated. These values were compared statistically with the median values in the Prisca program. Results: The current median values were low for free β-hCG and high for PAPP-A used in the first trimester screening test (p<0.001). It was observed that total β-hCG used in second trimester screening test was low except for the 19th week. Current median values were found to be high in all weeks for AFP and uE3 (p<0.001). Conclusion: There is a significant difference between the median values in the Prisca program and the median values calculated in our laboratory. Re-evaluation of risk analysis with current calculated median values will help clinicians to decide on the application of advanced diagnostic tests.

Özet

Amaç: Bu çalışmada, laboratuvarımızda yapılan prenatal tarama testlerinin güncel medyan değerlerinin hesaplanması ve risk hesaplamada kullandığımız paket programdaki medyan değerler ile karşılaştırılması hedeflenmiştir. Gereç ve Yöntem: Mayıs 2019-Mayıs 2020 tarihleri arasında İzmir ili ve çevresinden hastanemize yapılan başvurular sonucu 3264 birinci trimester tarama testi, 954 ikinci trimester tarama testi retrospektif olarak incelendi. Tüm parametreler İzmir Katip Çelebi Üniversitesi Atatürk Eğitim Araştırma Hastanesi Tıbbi Biyokimya Laboratuvarı’nda maternal serumdan çalışıldı. Elde edilen verilerin gebelik haftalarına göre güncel medyan değerleri ve medyan katları hesaplandı. Prisca paket programındaki medyan değerler ile istatistiksel olarak karşılaştırıldı. Bulgular: Güncel medyan değerlerinin; tüm haftalarda birinci trimester tarama testinde kullanılan free β-hCG için düşük, PAPP-A için yüksek olduğu, ikinci trimester tarama testinde kullanılan total β-hCG için 19. hafta dışında düşük olduğu görüldü (p<0.001). AFP ve uE3 için de tüm haftalarda güncel medyan değerler yüksek olarak bulundu (p<0.001). Sonuç: Prisca programı ile laboratuvarımızda hesaplanan medyan değerler arasında önemli bir farklılık görülmektedir. Güncel hesaplanan medyan değerler ile risk analizinin tekrar değerlendirilmesi; ileri tanı testlerinin uygulanması konusunda karar için klinisyenlere yardımcı olacaktır.
