Abstract

Choledochal cysts (CCs) are a congenital dilatation ofthe extrahepatic and/or intrahepatic bile duct whichare commonly associated with pancreaticobiliarymaljunction. Abdominal pain and recurrent attacks ofacute pancreatitis due to protein plugs or stones occurin 5-30% of patients with remnant intrapancreaticcholedochal cysts (IPCCs). The aim of this study wasto report a case with abdominal pain of protein plugsand stones due to remnant IPCCs after incompleteprior resection. A 49-year-old female patientpresented with abdominal pain on the epigastrium andright upper quadrant with intermittent fever, nausea, vomiting and weight loss that had lasted for more thansix months. One and a half years prior, the patient hadbeen diagnosed as a type I CC and undergone cystexcision with Roux-en-Y hepaticojejunostomy pluscholecystectomy at another hospital. Magneticresonance cholangiopancreatography (MRCP) markedcytic dilatation of the extrahepatic bile duct (remnantcyst) and an intraluminal mass. Endoscopic retrogradecholangiopancreatography (ERCP) was performedand protein plugs were removed from the distalcommon bile duct. The symptoms were partiallyrelieved and she had complaints repeatedly so it wasdecided to apply the Whipple procedure. Standardpancreaticoduodenectomy was performed. Patient’spostoperative period was uneventful and she wasdischarged on the 7th postoperative day

Endoskopik retrograd kolanjiopankreatografi (ERKP) ileremnant kistik dilate koledoktan protein tıkaçlarıçıkarıldı. Semptomlarında kısmi rahatlama olan veşikayetleri mükerrer kez tekrarlayan hastaya Whippleprosedürü uygulanmasına karar verildi. Hastaya standartpankreatikoduodenektomi ameliyatı yapıldı. Hastadaoperasyon sonrası komplikasyon gelişmedi ve hastapostoperatif 7. gün taburcu edildi
