Abstract

Objective: Alzheimer’s d isease ( AD) i s a progressive neurodegenerative disorder wi th incompletely understood molecular mechanisms. This study examined plasma SH3 and PX domains 2A (SH3PXD2A) concentrations in patients with AD, as this protein has not previously been directly investigated in AD or other neurodegenerative disorders. Material and Methods: A total of 45 patients w ith A D a nd 4 5 healthy controls were i ncluded. Plasma SH3PXD2A concentrations were measured using a commercial immunoenzymatic assay, and absorbance was recorded at 450 nm using a microplate spectrophotometer. Demographic variables, including age and gender, were also assessed. Results: Gender distribution was comparable between groups; however, participants wi th AD were significantly o lder t han controls. P lasma SH3PXD2A concentrations were significantly hig her in the AD group. In multivariable logistic regression analysis, bo th age an d pl asma SH3PXD2A concentration were independently associated with AD status. ROC analysis indicated moderate discriminatory performance for SH3PXD2A, with an area under the curve of 0.761 (95% CI: 0.657-0.866; p<0.001). A cut-off value of ≥3.120 ng/mL yielded 55.6% sensitivity and 97.8% specificity. The difference in SH3PXD2A concentrations remained significant after adjustment for age. Conclusion: Plasma SH3PXD2A concentrations were elevated in patients with AD and were independently associated with AD status after age adjustment. SH3PXD2A may represent a potential blood-based biomarker candidate in AD. Larger longitudinal studies incorporating detailed clinical characterization and mechanistic investigations are needed to determine its biological and clinical relevance.
