Abstract

Nodular scabies NS is a rare entity thought that hypersensitivity reaction to remnant of mites or antigens developing after scabies treatment. Pruritic nodules are mostly localized on genital region and scrotum. 21-years-old man admitted with itchy swellings on scrotum. He was diagnosed scabies 2 months ago and treated with %5 permetrin cream. Patient’s body lesions were improved but both scrotal lesions and pruritus got worsen. Dermatologic examination revealed multiple erythematous, smooth surfaced, mildly hard noduls on scrotum. Histopathologically; perivascular infiltration including eosinophils, plasma cells and PMNL were observed on superficial and deep dermis. The patient was diagnosed NS, pimecrolimus cream was started. Pruritus had regressed and lesions partially improved after a week. NS is a difficult to treat and rare disease in dermatologic practice. It may be confused with benign and malign other conditions clinically and histopathologically. NS must be thought in differential diagnosis of scrotal and penil nodular lesions
