Abstract

ABSTRACT Objective: The aim of this study was to determine the clinical and demographic characteristics and the frequency of concomitant atopic diseases in patients followed up in the Pediatric Allergy Outpatient Clinic with the diagnosis of food protein-induced allergic proctocolitis (FPIAP). Materials and Methods: The study included 108 cases diagnosed with food protein-induced allergic proctocolitis between January 2016 and December 2022 in the Pediatric Allergy and Immunology Outpatient Clinic of Trakya University Faculty of Medicine. Age, gender, prenatal, natal and postnatal information, genealogical information, FPIAP symptoms, diet, disease course and treatment, laboratory examinations, skin prick test results, food elimination information, age and duration of tolerance development were recorded retrospectively from the follow-up files. Results: The mean age at onset of symptoms was two months and the mean age at diagnosis was three months. 59 (54.6%) of the cases were boys and 49 (45.4%) were girls. At the time of symptom onset, 61% (66 cases) were exclusively breastfed, 28.7% (31 cases) were breastfed and formula fed, and 2.7% (3 cases) were exclusively formula fed. The prevalence of familial atopic disease was 39% (42 cases). After the elimination diet, bloody stools resolved within the first three days in 84.2% of the cases. The mean age at which tolerance developed was eight months. It was found that the elimination diet was discontinued in the home environment at an average of seven and a half months. At the time of diagnosis, 76% of the cases (82 cases) were accompanied by atopic disease. After the development of tolerance, the prevalence of atopic disease decreased to 40.7% and there was a significant decrease compared to the time of diagnosis (p<0.001). Conclusion: Food protein-induced allergic proctocolitis is common in infants with atopic background, the first findings are bloody stools, and the clinical picture improves significantly with food elimination. Food elimination was also found to improve comorbid atopic diseases.
