Abstract

Objective: This retrospective study aimed to evaluate and compare external apical root resorption between patient treated with clear aligners and those treated with fixed orthodontic appliances. Materials and Methods: This retrospective research comprised 40 patients treated at the Department of Orthodontics, Aydın Adnan Menderes University, between 2018 and 2024, and who met the inclusion criteria. Patients were categorized into two groups: clear aligner therapy (13 females, 7 males) and fixed orthodontic treatment (13 females, 7 males). Panoramic radiographs taken before and after treatment were analyzed using the modified classification system developed by Sharpe et al., and root resorption levels were assessed in the incisors, premolars, and molars of both jaws. Results: The average age of the patients was 18.8 ± 5.5 years in the clear aligner group and 15.4 ± 1.8 years in the fixed appliance group. Findings indicated that root resorption was typically mild in the two groups. Although the overall resorption scores were higher in the fixed appliance group, no statistically meaningful differences were detected in tooth-based comparisons (p>0.05). The most pronounced trend was detected in the maxillary lateral incisors (p=0.080). Conclusion: No meaningful difference in root resorption was detected between fixed orthodontic appliances and clear aligners. However, the fixed appliance group showed a slight tendency toward higher resorption. These results suggest that clear aligner therapy may represent a biologically safe alternative to fixed orthodontic treatment. Keywords: Root resorption, clear aligner, orthodontic appliance, panoramic radiography

Özet

Amaç: Bu retrospektif çalışmanın amacı, şeffaf plak tedavisi ile sabit ortodontik tedavinin eksternal apikal kök rezorpsiyonu üzerindeki etkilerini karşılaştırmaktır. Gereç ve Yöntem: Bu retrospektif çalışmaya, 2018–2024 yılları arasında Aydın Adnan Menderes Üniversitesi Ortodonti Anabilim Dalı’nda tedavi görmüş ve dahil edilme kriterlerini karşılayan toplam 40 birey dahil edilmiştir. Bireyler şeffaf plak grubu (13 kadın, 7 erkek) ve sabit ortodontik tedavi grubu (13 kadın, 7 erkek) olmak üzere ikiye ayrılmıştır. Panoramik radyografilerden elde edilen tedavi öncesi ve sonrası görüntüler, Sharpe ve arkadaşlarının geliştirdiği modifiye sınıflandırma sistemi kullanılarak analiz edilmiş; her iki çenede yer alan kesici, premolar ve molar dişlerde rezorpsiyon düzeyleri belirlenmiştir. Bulgular: Şeffaf plak grubunda ortalama yaş 18.8 ± 5.5 yıl iken, sabit ortodontik tedavi grubunda 15.4 ± 1.8 yıl olarak bulunmuştur. Bulgular, her iki tedavi grubunda da kök rezorpsiyonunun çoğunlukla hafif düzeyde olduğunu göstermiştir. Sabit ortodontik tedavi grubunda toplam rezorpsiyon skorları daha yüksek bulunmasına rağmen, diş bazlı karşılaştırmalarda istatistiksel açıdan anlamlı bir fark gözlenmemiştir (p>0.05). En belirgin farklılık eğilimi maksiller lateral kesicilerde gözlenmiştir (p=0.080). Sonuç: Şeffaf plak ve sabit ortodontik tedavi yöntemleri arasında kök rezorpsiyonu açısından anlamlı fark bulunmamıştır. Bununla birlikte, sabit tedavide hafifçe daha yüksek rezorpsiyon eğilimi gözlenmiştir. Bulgular, şeffaf plak tedavisinin biyolojik güvenlik açısından sabit ortodontik tedaviye alternatif bir seçenek olabileceğini göstermektedir. Anahtar Kelimeler: kök rezorpsiyonu, şeffaf plak, sabit ortodontik tedavi, panoramik radyografi
