Abstract

Heart failure is an important health problem since its incidence and prevalence is increasing year by year. Since symptom burden and mortality are high in heart failure, supportive and palliative care should be provided. However, very few patients are referred to palliative care services. In comparison with cancer patients, it is difficult to identify end of life care for patients with heart failure, because these patients are hospitalized when the signs of acute decompensation appear, and their symptoms decrease and functional status improve before they are discharged. Therefore, palliative care, which is a holistic approach aiming to improve patients’ quality of life, to detect and treat the attacks of the disease before they become severe, and to deal with patients’ physical, psychological, social, and mental health altogether during their care, should be integrated into heart failure patients’ care.

Özet

Kalp yetersizliği sık görülmesi ve görülme sıklığının her geçen yıl artması nedeniyle önemli bir sağlık problemidir. Kalp yetersizliğinde semptom yükünün ve mortalitenin yüksek olması, destekleyici ve palyatif bakım girişimlerinin sağlanmasını zorunlu hale getirmektedir. Ancak hastaların çok azı palyatif bakıma yönlendirilmektedir. Çünkü kanser hastalarının aksine kalp yetersizliği hastalarının hastalığın son safhasında akut dekompansasyon bulguları ile hastaneye yatması, taburculuk öncesinde semptomlarının azalması ve fonksiyonel durumlarının iyileşmesi nedeniyle yaşam sonu bakımın tanılanması zordur. Hastalarda yaşam kalitesinin iyileştirilmesini, semptomların kontrol altına alınmasını, hastalığın daha da ağırlaştığı atakların erken saptanmasını, tedavisini ve hastanın bakımında fiziksel, psikolojik, sosyal ve manevi sağlığın birlikte ele alınmasını hedefleyen bütüncül bir yaklaşım olan palyatif bakımın, kalp yetersizliği hastalarının bakımına entegre edilmesi önemlidir.
