Abstract

Background: Orthotopic liver transplantation (OLT) is characterized by profound intraoperative hemodynamic instability due to severe vasodilation, massive blood loss, major vascular clamping, and reperfusion syndrome. In this context, the extent to which invasive arterial pressure monitoring reflects true central hemodynamics is crucial for accurate titration of fluid and vasoactive therapy. This study aimed to compare PICCO2®-guided goal-directed fluid management via radial versus femoral arterial access in adult recipients undergoing living-donor OLT and to evaluate the impact of monitoring site on hemodynamic parameters, renal function, and short-term clinical outcomes. Materials and Methods: In this prospective observational study, 36 adult OLT recipients (18-65 years) were randomized to radial (n=18) or femoral (n=18) arterial monitoring. PICCO2® measurements-including systolic and mean arterial pressure, cardiac index, stroke volume variation (SVV), pulse pressure variation (PPV), and extravascular lung water index (ELWI)- were recorded at six intraoperative time points. Goal-directed therapy targeted MAP ≥65 mmHg; patients with SVV >10% received fluid boluses, whereas those with SVV ≤10% received norepinephrine-based vasoconstrictor therapy. Results: Groups were comparable in demographics, surgical duration, fluid and blood product administration, vasopressor requirements, creatinine trends, and ICU stay. Femoral monitoring yielded higher systolic pressures during early anhepatic and clamping phases, whereas MAP, cardiac index, SVV/PPV, ELWI, postoperative renal function, and mortality rates showed no significant intergroup differences. Conclusions: Findings suggest that radial and femoral arterial monitoring provide similar clinical outcomes during PICCO2®-guided goal-directed therapy in OLT. However, the higher systolic pressures observed with femoral monitoring during critical phases may offer a physiological advantage by supporting end-organ perfusion.
