TÜRKİYE’DE ÇOCUK CİNSEL İSTİSMARI: GÖZDEN GEÇİRME ÇALIŞMASI

Özet
Cinsel istismar, tüm dünyada önemli birsorun olmaya devam etmektedir. Ülkemizde de sonyıllarda bu konu ile ilgili yapılan çalışmaların sayısıartmaktadır. Bu çalışmada; Türkiye’nin farklıbölgelerinde cinsel istismar mağdurları üzerindeyapılmış olan, olguların sosyodemografik veistismara ait özelliklerini içeren araştırmayazılarının gözden geçirilmesi ve güncel bilgilerinortaya konması amaçlanmıştır.YöntemTürkiye’de son 10 yılda, cinsel istismarüzerine yapılmış çalışmalar ‘istismar’, ‘cinselistismar’ anahtar kelimeleri ile Pubmed, GoogleAkademik, Scoopmed, Türk Psikiyatri Dizini,Türkiye Atıf Dizini ve Medline arama motorlarıkullanılarak taranmıştır. Çalışmamıza polikliniğebaşvuran adli olguların dökümünü yapan 20çalışma alınmıştır.BulgularÇalışmaların geneline baktığımızdaolguların %50’den fazlası 12-18 yaş aralığındaydı.Ülkemizdeki çalışmalarda kız/erkek oranı en düşük1,3, en yüksek 6,6idi. Parçalanmış aile oranıtoplum ortalamasına göre daha yüksekti. Cinselistismar mağdurları istismarın türü yönündenincelendiğinde penetrasyona (oral, anal, vaginal)maruziyet oranı oldukça yüksek olduğugözlemlenmiştir. İstismarcıya olan yakınlıkaçısından değerlendirildiğinde batı ülkelerinebenzer oranlar saptanmıştır.SonuçBu çalışmalarda genel olarak kayıtlarınyetersiz tutulduğu saptanmıştır. Aynı zamandakullanılan tanımlamalarda da farklılıklar olduğugözlenmiştir. Ülkemizde 3 yaş altı mağdurolmaması ve batı ülkelerine görepenetrasyonamaruziyet oranının yüksek olmasıdikkat çekmiştir. Bu gözden geçirme çalışmasındanelde edilen bilgiler ışığında bazı önerilerinyapılması uygun görülmüştür
Abstract
Sexual abuse continues to be a major problem inthe world. In our country the numbers of thesetypes of studies are increasing in recent years. Inthis study our aim is to review literature whichconcerns with sociodemographic characteristics ofsexual abuse victims and types of sexual abuse.MethodsThe studies on sexual abuse conducted within thelast 10 years in Turkey were scanned using thekeywords ‘abuse’ and ‘sexual abuse’ through thePubmed, Google Academic, Scoopmed, TurkishPsychiatry Index, Turkish Citation Index, and theMedline search engines. Twenty studies focussingon the criminal cases referred to our clinic wereincluded in our study and data obtained through theretrospective scan of the abuse victims’ records invarious regions of our country were analysed.ResultsWhen we looked at the cases in general, the victimwas between 12 and 18 years of age in more than50% of the cases. In the studies from our country,the female/male ratio was 1.3 at the lowest and 6.6at the highest. The ratio of broken familiesobserved in the studies conducted in our countryvaried between 14.5% and 28.6%. This result wasover the average rate in the general society. Whenthe victims of sexual abuse were evaluated in termsof the type of abuse, the ratio of (oral, anal, orvaginal) penetration was observed to be rather high.The analysis in terms of the relationship with theabuser resulted in similar rates to the westerncountries.ConclusionThese studies also show that the records aregenerally far from complete. Also, a lack ofstandardisation was observed in terms of thedefinitions used. The observation that none of thevictims were below the age of 3 and the rate ofpenetration in our country was higher incomparison to the western countries were pointsworth considering. In the light of the informationobtained from this review study, certainrecommendations are deemed to be necessary.
