LOMBER DİSK HASTALARINDA AŞIRI AKTİF MESANE SENDROMUNUN DEĞERLENDİRİLMESİ

Özet
AMAÇ: Kliniğimize başvuran ve lomber diskhastalığı tanısı alan fakat idrar şikayeti olmayanolgularda, aşırı aktif mesane bulgusunun varlığınıincelemektir.MATERYAL VE METOD: Nöroşirurjipolikliniğine Haziran2013 -Eylül 2013 tarihleriarasında başvuran ve klinik ve radyolojik olaraklomber disk hernisi tanısı alan 50 hasta, ürolojipolikliniği ile konsülte edilerek aşırı aktif mesanevarlığı açısından uluslarası ‘’aşırı aktif mesanedeğerlendirme formu’’ nun Türkçe validasyonu iledeğerlendirildi. Sekiz ve üzeri skor aşırı aktifmesane açısından müsbet kabul edildi.BULGULAR: Hastaların yaş ortalaması 47 ± 2 idi.Hastalar Macnab sınıflandırmasına göredeğerlendirildiğinde bulging oranı %56 ,protüzyonoranı %32 , ektrüzyon oranı %6 ve sekestre diskoranı %6 olarak tespit edildi. Kadın hastaların%56.5 'inde, erkek hastaların %33.3'ünde aşırı aktifmesane tanısı konuldu. Diskopatinin seviyesinegöre L4-5 seviyesinde seviyesinde daha yüksekoranda aşırı aktif mesane tanısı konduğu, Macnabsınıflandırmasına göre ise en yüksek aşırı aktifmesane semptom skorunun protüzyon izlenenhastalarda olduğu ( %50) saptandı.SONUÇ: Lomber disk hernisi tanısı alanhastalarda şikayeti olmasa da değişik derecelerdeürolojik problemler olabilir. Hem tıbbi hem desosyal sorunlara sebebiyet veren aşırı aktif mesanedurumu lomber disk hastalığı tanısı alan olgulariçin ağrı, motor ve duyu defisiti açısındansorgulanırken dikkat edilmesi gereken bir diğertıbbi sorundur.
Abstract
The objective of the study is toanalyze the association of overactive bladder in thepatients with lumbar discopathy at various stageswithout complaint of blader disfontions atoutpatient clinic.MATERIALS AND METHODS : 50 patients whowere admitted to outclinic with low back paincomplaint and whose diagnoses of lumbar dischernia were radiologically verified were assessed interms of overactive bladder by also consulting withthe urology polyclinic. The lumbar disc hernia wasradiologically performed according to Macnab’sclassification which calassificated clinical evidencewith radiological anomalys . The existence ofoveractive bladder (OAB) was correlated by meansof the assessment .The OAB score of 8 and abovewas regarded / accepted as high, in other words,positive.FINDINGS : The average age of these patientswere 47, the proportion of female was %46 andmen was %54, the bulging rate according toMacnab’s classification was %56, the protrusionratio was %32, the extruded disc ratio was %6, andthe scestre dic ratio was %6.All these patients wereclassified according to the overactive bladder(OAB) assessment form. As a result, the OAB scorewas monitored according to female ratio of%56.5and male %33.3, and according to the level ofdiscopathy L4-5 the highest at its level, while thehighest OAB score (protrusion) according toMacnab’s classification was monitored as %50.RESULT : There may be urological problems inpatients with lumbar disc hernia at differentiatingdegrees. These problems, as well giving rise tomedical problems, may also lead to socialchallenges. The clinician, while questioning thepain, motor and sensory deficit in terms of lumbardisc hernia, should also take into consideration theurological problems indicating close/frequentassociation at a considerable amount.
