Özet
Amaç: Bu çalışma, bir nöroloji yoğun bakım ünitesinde 9 yıl önce sunulan hemşirelik hizmetlerinin süreleri ile bugünkü hemşirelik hizmetlerinin sürelerini karşılaştırmayı, ünitedeki mevcut durumu saptamayı ve yoğun bakım ünitelerinde yeterli sayıda/nitelikli hemşirelerin istihdam edilmesinin önemini vurgulamayı amaçlamaktadır. Gereç ve Yöntemler: Çalışmamız, yoğun bakımda çalışan hemşirelerin 2001 ve 2010 yılında bir mesai saati içinde verdiği hemşirelik hizmetlerinin sürelerinin, yoğun bakımda çalışmış deneyimli bir hemşire (gözlemci) tarafından hemşirelerden habersiz olarak ara verilmeden, sü- rekli gözlemlenerek kayıt edilmesiyle yapılmıştır. Çalışmamızda hemşirelerin teslim verme- teslim alma süreleri, malzeme hazırlığı, hasta bakımı, takibi, tedavisi ve kayıtları için geçen süreler ile el antiseptiği kullanımı, el yıkama ve eldiven değişim sayıları, 2001 yılında 2 hemşire gündüz 2 hemşire gece nöbet ekibinden, 2010 yılında da aynı şekilde 2 hemşire gündüz 2 hemşire gece nöbet ekibinden olmak üzere toplam 8 farklı yoğun bakım hemşiresinde farklı günlerde gözlemlenerek kayıt edilmiştir. Gözlemler sonucu elde edilen bulgular dakika olarak hesaplanıp aritmetik ortalamaları alınmıştır. Bulgular: Hemşirelerin hasta teslim alma ve teslim verme sürelerinin 2010 yılında artmış olduğu; hasta bakımı, takip alma ve tedavi yapma sürelerinde, 2010 yılı gündüz mesaisinde, 2001 yılında ise gece mesaisinde hemşirelerin daha fazla zaman harcadıkları belirlendi. Hemşirelerin 2010 yılında daha fazla el antiseptiği kullandığı, eldiven değiştirdiği ve el yıkadığı saptandı. 2001 yılında dosya kayıtları ortalamasının daha fazla olduğu; 2010 yı- lında ise bilgisayar kayıtlarıyla birlikte kayıt işlemlerine ayrılan sürenin artmış olduğu saptandı. Sonuç: Teknolojik gelişmelerin yoğun bakım ünitelerinde hemşirelik hizmetlerine ayrılan süreyi azalttığı belirlendi ve bu teknolojiyi kullanabilmek için yoğun bakım ünitelerinde yeterli sayıda/nitelikli hemşireler istihdam edilmeli, eğitimlere önem verilmelidir.
Abstract
Objective: This study aimed to compare the durations of today’s nursing service functions with that of 9 years ago in a neurology intensive care unit, to determine the present aspects of the unit , to improve the quality of the services and in order to emphasize the importance of the employment of sufficient number and qualified nurses. Material and Methods: In our study, the durations of the nursing services of nurses ,who has worked in intensive care units, in a working hour in different days and shifts in 2001 and in 2010 were lasting observed and recorded during all shift without informing the nurses by an experienced nurse, who has worked in an intensive care unit. In our study,the patient reception-delivery times, material preparation, patient care, pursuit , treatment and recording durations and hand antiseptic application, hand washing and glove changing counts were recorded in a total of 8 different intensive care unit nurses in different days in 2001 (2 nurses between 08.00-16.00 in the day time shift) and in 2010 (2 nurses between 16.00-09.00 in the night shift). Durations obtained by observations were recorded as minutes and the average values were computed. Results: In our study, ıt was determined that nurses’ the patient reception and delivery times to be increased in 2010 ; patient care, making follow-up and treatment times; nurses spent more time in the night-time shift in 2001 and in the day shift in 2010. In 2010, it was found out that the nurses used more hand antiseptic, changed more gloves and washed more often their hands and the average of file records was higher in 2001, with computer records the time, which allocated the records increased in 2010. Conclusion: In this study, it was determined that technologic developments reduced the time of the nursing service in intensive care units and to use this technology ,the employment of sufficient number and qualified nurses ,to give importance to education, to research technological development of nurses in intensive care units are recommended to follow.
