Güvenlik raporlama sistem bildirimlerinin yıllara göre değerlendirilmesi (2016-2017-2018)

Özet
Giriş ve Amaç: Güvenlik Raporlama Sistemi (GRS), sağlık tesislerinde hasta ve çalışan güvenliğini tehdit eden, gerçekleşen veya ramak kala olayların bildirilebileceği bir platformdur. Çalışmanın amacı, Sağlık Bilimleri Üniversitesi Dışkapı Yıldırım Beyazıt Eğitim ve Araştırma Hastanesinde meydana gelen, gerçekleşen veya ramak kala olayların bildirilmesi durumu, yapılan bildirimlerin başlık olarak değerlendirilmesi ve 2016-2017-2018 yılları bildirimlerinin karşılaştırılmasıdır.Gereç ve Yöntem: Veriler sağlık çalışanlarının hastanenin herhangi bir alanında bulunan bilgisayardan yapılan olay bildirimlerinin değerlendirilmesi ile toplanmıştır. 2016, 2017, 2018 yıllarında yapılan bildirimlerin analizleri yapılmıştır.Bulgular: Çalışma bulgularına göre; 2016 yılında 31 olay bildirimi yapıldığı, bu bildirimlerin tamamının gerçekleşen olay olduğu, 2017 yılında 50 olay bildirimi yapıldığı, bu bildirimlerin 48’inin gerçekleşen olaylar, 2 tanesinin ise ramak kala olay bildirimi olduğu, 2018 yılında ise, 83 olay bildirimi yapıldığı bu bildirimlerin 47 tanesinin gerçekleşen olay, 36 bildirimin ramak kala olay bildirimi olduğu görülmektedir. Sistem bildirimleri içeriksel olarak incelendiğinde özellikle 2018 yılında bildirimlerin büyük bir kısmının ramak kala olaylar olduğu görülmüştür. Sonuç: Sonuçlar değerlendirildiğinde çalışanlarda gerçekleşen ve ramak kala olay bildirimlerini korkmadan yaptıklarını ve yıllara göre bildirimlerin artış gösterdiği görülmüştür. Bunun dışında hastanede olay gerçekleşmeden, risk gördükleri konularda bildirim yapma kültürü oluştuğu sonucuna ulaşılmıştır. Bu sonuç hastane için oldukça önemlidir. Olay gerçekleşmeden bildirim yapılması hastalar veya çalışanlar zarar görmeden, tespit edilen aksaklıkların giderilmesi ve önlem alınması anlamına gelmektedir. Dolayısıyla ulaşılan sonuç, hastanede GRS eğitimlerinin etkinliğini, kurulan sistemin başarısını ve çalışanlarda kültürün oluştuğunu göstermektedir.
Abstract
Introduction and Purpose: The Safety Reporting System(SRS) is a platform where health events can be reported to threaten patient and employee safety, occur or occur. The aim of this study is to report the occurrence of events occurring or occurring at the Ministry of Health Sciences, Dışkapı Yıldırım Beyazıt Training and Research Hospital, evaluating the reports made as a title and comparing the declarations of 2016-2017-2018 years.Material and Method: The data were collected by evaluating the incident reports made by the healthcare workers in any area of the hospital. The reports made in 2016, 2017 and 2018 were analyzed.Findings: According to the study findings; In 2016, 31 events were reported and all of these notifications were realized. In 2017, 50 events were reported, 48 of them were reported events and 2 of them were reported. In 2018, it is seen that 83 of these notifications, 83 of which were notified, were the events of 47 events and 36 notifications of events. When the system notifications were analyzed contextually, it was observed that most of the notifications were recorded in 2018. Result: When the results were evaluated, it was seen that the employees realized the reports of the incident and they did not fear and the reports showed an increase according to the years. In addition, it was concluded that there was a culture of notifying the risk issues before the event occurred in the hospital. This result is very important for the hospital. Notification before the event occurs means eliminating any malfunctions and taking precautions before patients or employees are harmed. Therefore, the result shows that the effectiveness of the GRS training in the hospital, the success of the established system, and the culture are formed in the employees.
