IDA JEAN ORLANDO’NUN ETKİLEŞİM MODELİNE GÖRE KRONİK BÖBREK YETMEZLİĞİ TANILI HEMODİYALİZE GİREN HASTANIN İNCELENMESİ ve HEMŞİRELİK BAKIMI: VAKA SUNUMU

Özet

Bu olgu, kronik böbrek yetmezliği tanısı ile hemodiyaliz tedavisi gören bir hastanın bakımında Orlando’nun Etkileşim Modelini kullanarak, hemşireleri model kullanımına yönlendirmek, hastaya özgü hemşirelik girişimlerini geliştirmek ve hemşirelik bakımını sistematik olarak sunmak amacı ile planlanmıştır. Vaka 66 yaşında evli, 3 çocuk sahibi bir annedir. 57 yaşındayken ağız kuruluğu, sık sık idrara çıkma belirtileri ile iç hastalıkları polikliniğine başvuran hastadır. Yapılan tetkikler sonucunda Diabetus Mellitus tanısı konulmuş. 59 yaşında baş ağrısı belirtisi ile acil servise tekrar başvuran hastaya hipertansiyon tanısı konulmuştur. 64 yaşında tekrarlayan halsizlik, yorgunluk, öksürük ve enfeksiyon belirtileri ile hastaneye başvurmuş. Kronik böbrek yetmezliği tanısı ile takibe alınan hastaya, santral kateter takılarak hemodiyaliz tedavisi başlamış. Bu süreçte hastanın sol kolundan fistül açılıp hemodiyaliz tedavisi devam etmiştir. Hemodiyaliz tedavisi sürecinde hastada yorgunluk, halsizlik, el ve ayaklarda kramp, kaşıntı, ağrı, uykusuzluk şikayetleri oluşmuş. Aynı zamanda hasta diyetine uymakta zorlandığını, iyi bir eş olamadığını ve ölüm korkusu yaşadığını belirtmiştir. Tedavi sürecinde hasta sürekli olumsuz semptomlarla baş etmek zorunda kalmıştır. Hastanın KBY tanısı almış olması, özel ve zorlu bir tedavi sürecini yaşıyor olması birey için ciddi bir fizyolojik ve psikolojik gereksinimi beraberinde getirmektedir. Bu nedenle birey sistematik bir hemşirelik bakımına ve profesyonel iletişimle birlikte rehberliğe ihtiyaç duymaktadır. Orlando’nun ‘’etkileşim teorisi’’ne göre, hemşirelik süreci; hastanın davranışı, hemşirenin tepkisi ve hastanın yararına şekillendirilmiş hemşirelik girişimlerinin birbirlerini etkilemesi olarak belirtilmiştir. Orlando’nun "etkileşim teorisinde"; hasta ile bu çerçevede kurulan iletişim sürecinde sistematik bir hemşirelik bakımı planlanmış, geliştirilmiş ve uygulanmıştır. Süreç sonunda hastanın sözel ve sözel olmayan davranışlarında olumlu geri bildirim alınmıştır. Sonuç olarak; Orlando’nun etkileşim teorisi, hasta ile iletişim kurma, hasta tepkilerini anlama, gereksinimlerini belirleme ve bakımı uygulamada kullanılabilir.

Abstract

This case was planned with the aim of directing nurses to model use by using Orlando's Interaction Model in the care of a patient undergoing hemodialysis treatment with the diagnosis of chronic renal failure, developing patient-specific nursing interventions and systematically presenting nursing care. The case is a 66-year-old married mother with three children. The patient was admitted to the internal diseases outpatient clinic with symptoms of frequent dryness when he was 57 years old. Diabetes mellitus was diagnosed as a result of the examinations. 59 years old went to the emergency room with a symptom of headache. The patient was diagnosed with hypertension. At the age of 64 years, she was admitted to the hospital with symptoms of weakness, fatigue, cough and infection. Hemodialysis treatment was started with central catheter insertion. In this process, fistula was opened from the left arm of the patient and hemodialysis treatment was started. Hemodialysis treatment in patients with fatigue, fatigue, cramps, itching, pain, insomnia and hands occurred in the hands. She also stated that she had difficulty complying with the diet of the patient, had not been a good wife and had fear of death. During the course of the treatment, the patient had to cope with negative symptoms. The fact that the patient is diagnosed with CRF, is experiencing a special and challenging treatment process brings with it a serious physiological and psychological need for the individual. Therefore, the individual needs a systematic nursing care and professional communication and guidance. According to Orlando's ’interaction theory te, the nursing process; the behavior of the patient, nurses' response and shaped nursing interventions to benefit the patient are mentioned as affecting each other. According to Orlando's "interaction theory"; A systematic nursing care was planned, developed and implemented in the communication process established with the patient. At the end of this process, positive feedback was obtained in verbal and nonverbal behavior of the patient. As a result; Orlando’s interaction theory can be used to communicate with patients, understand patient responses, identify needs, and practice care.
