Özet

Amaç: Yenidoğan sarılığı, etyolojisi çok çeşitli olup bazanda hiçbir nedenin saptanamadığı doğum sonras ı yenidoğanın ilk haftada kar şılaştığı önemli sorunlardan biridir. Hastaların kliniğide farklılık arzedip erken tan ı konulmazsa ölümle sonuçlanabilir. Yenido ğan sar ılığında etiyolojik nedenlerden biri olan Glukoz-6-fosfat dehidrogenaz (G6PD) eksikliği, ülkemizde yenidoğan sar ılığı nedenleri arasında yer almaktadır. Bu çalışma ile hastanemizde sarılık nedeniyle izlenen yenidoğanlarda G6PD enziminin sarılığa etkisini incelemek amaçlanmıştır. Gereç ve Yöntem: Çalışmada Elazığ Şehir Hastanesinde yenidoğan servisinde yenidoğan sarılığı tanısıyla takip edilen 100 bebek retrospektif olarak incelendi. Hemogram, G6PD, bilirubin ve yap ılan tüm kan tetkikleri laboratuvarımızda ve standart koşullarda gerçekleştirildi. İstatistiksel değerlendirmede total bilirubin seviyesi baz al ındı. Hastaların total bilirubin seviyeleri 12 mg/dL altındaki seviyelere inene kadar her gün günde en az bir kez bakıldı. Bulgular: Çalışmamızda sar ılık tanısı konulan 100 bebekte en sık sarılık nedeni olarak 31(%31)’inde ABO uyuşmazlığı, 10(%10)’da G6PD eksikliği ve 8(%8)’in de prematüriteye ba ğlı geliştiği tespit edildi. G6PD eksikliği olan bebeklerde total bilirubin seviyelerinin enzim eksikliği olmayan bebeklere göre dikkat çekici bir şekilde uzun sürede düştüğü görüldü. Hastaların di ğer bakılan parametrelerinde anlamlı fark tespit edilmedi. G6PD eksikliği olan bebeklerin 52(%52)’si erkek, 48(%48)’i k ız bebekti, istatistiksel olarak anlamlı bir fark tespit edilmedi. Sonuç: Yenidoğanda sarılık etiyolojisi araştırılırken bebeklerde mutlaka G6PD düzeylerine de bakılması uygun olacaktır ve G6PD düzeyi düşük bebeklerde sar ılığın uzun sürede düzelebileceği göz önünde bulundurulmalı ve gerekli tedavi bu şekliyle planlanmalıdır.

Abstract

Introduction: Neonatal jaundice is one of the important problems encountered by the new born in the first week after birth, the etiology of which is very diverse and sometimes no cause can be detected. It can result in death if the patients differ in their clinics and are not diagnosed early. Glucose-6- phosphate dehydrogenase (G6PD) deficiency, which is one of the etiological causes of neonatal jaundice, is among the causes of neonatal jaundice in our country. In this study, it was aimed to examine the effect of G6PD enzyme on jaundice in newborns followed up in our hospital for jaundice. Materials and Methods: In the study, 100 babies who were followed up with the diagnosis of neonatal jaundice in the neonatal ward of Elazig City Hospital were retrospectively analyzed. Hemogram, G6PD, biliru bin and all blood tests were performed in our laboratory and under standard conditions. Statistical evaluation was based on total bilirubin level. The total bilirubin levels of the patients were measured at least once a day until they decreased to levels below 12 mg/dL. Results: In our study, the most common causes of jaundice in 100 babies diagnosed with jaundice were ABO incompatibility in 31(31%), jaundice due to G6PD deficiency in 10(10%) babies. It was observed that total bilirubin levels in babies with G6PD deficiency decreased significantly over a longer period of time compared to babies without enzyme deficiency. No significant difference was found in the other parameters of the patients. Of the babies with G6PD deficiency, 52(52%) were boys and 48(48%) were girls. Conclusion: While investigating the etiology of jaundice in newborns, it would be appropriate to check the G6PD levels in babies, and it should be considered that jaundice may improve in a long time in babies with low G6PD levels, and the necessary treatment should be planned in this way.
