Özet

Baş boyun bölgesinde travma ve tümör rezeksiyonu nedeniyle oluşan defektlerin onarımında çeşitli flepler kullanılmaktadır. Bu çalışmada 1994 - 2001 yılları arasında serbest serratus kas flepleriyle baş boyun bölgesinde defekt rekonstrüksiyonu yapılan 13 olgu değerlendirildi. Defektler; olguların dokuzunda tümör rezeksiyonu nedeni ile dördünde ateşli silahla yaralanma sonucu meydana gelmişti. Olguların sekizine serratus anterior kas flebi uygulandı. Üç olguda serratus anterior kas-deri flebi ile onarım yapıldı. Bir olguda serratus anterior kası skapular kemik flebiyle (angular arter pediküllü) birlikte, diğer bir olguda ise kostal kemikle kompozit olarak hazırlandı. Hastalar ameliyat sonrası ortalama 18 ay izlendiler. Bir olguda flep hematom nedeniyle kaybedildi. Diğer olgularda sonuçlar estetik ve fonksiyonel açıdan iyi olarak değerlendirildi. Geniş ve ince bir kas olan serratus anterior kası, defektleri örterken yüz konturunu fazla bozmamaktadır. Ayrıca pedikülünün uzun olması ve skapular bölgenin komşu dokularıyla çeşitli kompozit flepler hazırlama olasılığı diğer üstünlükleridir.

Abstract

Various types of flaps have been used for reconstruction of defects after trauma or tumor resection in head and neck region. in this study 13 patiens who were reconstructed with free serratus anterior muscle flaps in head and neck region between 1994-2001 are evaluated. in 9 patients the defects were related with tumor resection and in 4 patients with gunshot wounds. in 8 patients serratus anterior muscle flaps, in 3 patients muscle-skin flaps and in 2 patients muscle-bone flaps were performed. The mean follow up period was 18 months. in one patient the flap was lost because of hematoma. in the other cases the results were evaluated aesthetically and functionally good. The serratus anterior muscle with is a large and thin muscle does not change the facial contour very much when covering the defects. Futhermore it s long pedicle and the possibility of construction with local tissue as composite flaps are the other advantages.
