Özet

Lomber disk cerrahisi beyin cerrahisi pratiğinin en sık uygulanan cerrahi yöntemidir. Cerrahi teknik ve merkezden bağımsız olarak ilk kez cerrahi yapılacak olgularda başarı oranı %80-85'tir. Lomber diskektomi sonrası şikayetlerin geçmemesi "Cerrahi Geçirmiş Bel Sendromu" olarak tanımlanmaktadır. Nüks disk hernileri de bu klinik durumun en sık gözlenen şeklidir ve %7 ile %26 oranında görülür. Nüks disk hernisi; ameliyat edilen aynı seviyede, aynı tarafta veya karşı tarafta ameliyattan sonraki 6 aylık ağrısız süre içinde yeniden herni gelişmesidir. Birçok predispozan faktör bildirilse de nüks gelişiminden en çok uygulanan cerrahi teknik sorumlu bulunmuştur. Yaş, cinsiyet, sigara, alkol, ağır kaldırma rekürrens riskini arttıran faktörler olarak bildirilmiştir. Literatürde farklı oranlar bildirilse de lomber diskektomi olgularının %10'una revizyon cerrahisi uygulanmakta ve hangi yaklaşımın daha uygun olduğu ile ilgili bir konsensus bulunmamaktadır.

Abstract

Lumbar disc surgery is the most common surgical procedure in neurosurgery practice. Regardless of the surgical technique and center, the success rate for patients who underwent surgical intervention for the first time are reported in the literature as 80-85%. Failure of patients after the lumbar discectomy is defined as "Surgically treated waist syndrome". Recurrent disc herniation is the most common postoperative cause of this clinical condition and has been reported to occur in up to 7% to 26% of cases. Recurrent disc herniation has been defined as the presence of a herniated disc at the same level, ipsilateral or contralateral, in a patient who has experienced a pain-free interval of at least 6 months since surgery. Although multiple factors are responsible for the development of recurrent lumbar disc, it is known that the surgical technique used is most responsible for the development of recurrence. Age, gender, smoking habits, obesity, alcohol consumption, sports activities and heavy lifting have been reported to increase the risk of recurrence. Although various numbers are reported, it can be said that 10% of the patients who have undergone lumbar discectomy go to revision surgery in the long-term and there is no consensus for which approach is better in the literature.
