Migrenli Olgularda Faktör V Leiden, Protrombin G20210A ve Metilentetrahidrofolat Redüktaz C677T Mutasyon Sıklığı

Özet
: Migren iskemik tip serebrovasküler olaylar için tek başına risk faktörü olarak kabul edilmiştir. Migren iskemik inme birlikteliği-- nin fizyopatolojisi tam olarak anlaşılamamıştır. Migren hastalarında tromboza eğilim yaratan genetik faktörler patogeneze ışık tutma-- sı amacıyla daha önce de araştırılmıştır, fakat fikir birliğine varılamamıştır. Bu çalışmada migrenli hastalarda tromboza eğilim yaratan faktör V leiden (FVL), protrombin (Pt) GZOZ1OA ve metilentetrahidrofolat redüktaz (MTHFR) C677T mutasyonu sıklığı araştırılmıştır. Hastalar ve Yöntem: Başkent Üniversitesi Hastanesi Nöroloji Polikliniğine başvuran 15--55 yaş arası Uluslararası Baş Ağrısı Derneği (IHS) tanı kriterlerine göre migren tanısı konulan ve sistemik hastalığı olmayan 160 hastadan izole edilen genomik DNA, FVL, Pt GZOZlOA ve MTHFR C677T gen polimorfizmleri açısından değerlendirildi ve Türk popülasyonuna ait sağlıklı kontrol grubu ile kıyaslandı. Bulgular: FVL mutasyonu 160 migrenli hastanın 155 (%96.9)'inde homozigot normal 5(%3.1)'inde ise heterozigot olarak saptandı, hastaların hiçbiri homozigot mutant değildi. Kontrol grubunda ise heterozigot birey oranı %98 olmasına rağmen iki oran arasındaki fark istatistiksel olarak anlamlı bulunmamıştır (p> 0.05). Normal Türk popülasyonu ile yapılan çalışmanın sonucuna benzer şekilde bi-- zim çalışmamızda da hiç homozigot mutant birey saptanmamıştır. MTHFR C677T mutasyonu için 160 migrenli hastada heterozigot hasta sayısı 39 (%24.4), homozigot mutant hasta sayısı ise 8 (%5) iken; kontrol grubunda 37 (%34.9) kişi heterozigot ve 6 (%5.6) homozigot mutant bulunmuştur. Oranlar arasında istatistiksel olarak anlamlı fark saptanmamıştır (p= 0.15). Pt G20210A mutasyonu için 160 migrenli hastadan 3 (%1.9)'ü heterozigot iken, kontrol grubunda heterozigot birey sayısı (%2.9) olarak saptanmıştır. Bu oranlar arasında da istatistiksel olarak anlamlı bir fark saptanmamıştır (p= 0.420). Yorum: Çalışmamız trombotik risk faktörlerinden FVL, Pt G20210A ve MTHFR C677T gen mutasyonlarının popülasyonumuzda migren patogenezi ile ilişkili olmadığını göstermektedir. Toplumlar arası farklılık vardır ve bu fark muhtemelen etnik köken ayrılığına bağlıdı
Abstract
Migraine is an independent risk factor for ischemic stroke, but its pathophysiology is still unclear. Genetic factors that pre— dispose patients to thrombosis have been studied in patients with migraine to highlight the pathogenesis, but the results remain cont» roversial. In this study, the frequencies of factor V Leiden (FVL), prothrombin (Pt) GZOZlOA and methylenetetrahydrofolate reducta— se (MTHFR) C677T mutations were investigated. Materials and Methods: One hundred and sixty patients aged of 15 to 55 years with no history of systemic disease and who had been diagnosed as migraine according to the International Headache Society (IHS) diagnostic criteria at Baskent University Hospital Neurology Outpatient Clinics were investigated for FVL, Pt GZOZ1OA and MTHFR C677T mutations from their genomic DNA, and the results were compared with those of healthy controls. Results: One hundred and fifty five (96.9%) of 160 migraine patients were homozygote normal, 5 (3.1 %) were heterozygote and no» ne of them were homozygote mutant for FVL. The control group had 9.8% heterozygote individuals but the difference between the percentages was not statistically significant (p> 0.05). There were no homozygote mutant individuals in the Turkish population study in normal subjects like our study. Thirty nine (24.4%) of 160 migraine patients were heterozygote and (5%) were homozygote mu— tant for MTHFR C677T. The control group had 37 (34.9%) heterozygote and (5.6%) homozygote mutant individuals. The differen— ce between the percentages was not statistically significant (p= 0.15). Three (1.9%) of 160 migraine patients were heterozygote and 6 (2.9%) of the control group were heterozygote mutant for Pt 62021 OA mutation. The control group had 37 (34.9%) heterozygote and (5.6%) homozygote mutant individuals. The difference between the percentages was not statistically significant (p= 0.420). Conclusion: Our study indicates that FVL, Pt GZOZ1OA and MTHFR C677T gene mutations, which are considered as risk factors for thrombosis, were not found to be associated with migraine pathogenesis in our population. The differences between populations probably depend on variations in ethnic origin
