Endometriomalarda second look laparoskopi sonuçlarımız

Özet
Amaç: Endometrioma olgularını sekond-look laparoskopi ile değerlendirmek Materyal ve Metod: Hastanemizde endometrioma nedeniyle operatif laparoskopi uygulanan ve 3 ay sonra sekond-look laparoskopi (ikinci bakı laparoskopisi) ile değerlendirilen 24 hastanın kayıtları retrospektif olarak incelendi. Bulgular: Çalışmaya alınan hastaların 10’u (%41,7) primer infertil, 3’ü (%12,5) sekonder infertildi. Hastaların 15’inde (%62,5) unilateral endometrioma, 6’sında (%25) bilateral endometrioma mevcuttu. İlk laparoskopi sırasındaki ortalama American Fertility Society (AFS) skoru 40,35± 21,64 idi. Postoperatif dönemde 20 (%83,3) hastaya 3 ay süreyle Gonadotropin Releasing Hormone (GnRH) analogu verildi. Second-look laparoskopisinde (SLL) tespit edilen ortalama AFS skoru 15,61± 16,41 olarak bulundu. Birinci ve ikinci laparoskopilerde tespit edilen AFS skorları karşılaştırıldığında , iki skor arasında istatistiksel olarak anlamlı fark bulundu ( p< 0,0005; paired t test).Sonuçlar:Endometrioma tedavisinde cerrahi ve medikal tedavinin kombine kullanımı ile iyi sonuçlar elde edildiği SLL ile gösterilmiştir.
Abstract
Objective: To evaluate the treatment results of endometriomas by second look laparoscopy (SLL). Materials and Methods: The medical records of 24 patients with endometriomas who had been operated by laparoscopy were evaluated, and results of SLL 3 months after the first operation were compared. Results: Ten of the patients (41.7%) had primary and 3 of 24 (12.5%) had secondary infertility as diagnosis. Fifteen patients (62.5%) had unilateral and 6 patients (25%) had bilateral endometriomas. The mean American Fertility Society (AFS) scores of endometriosis were 40,35±21,64 at the first operation. Twenty (83.3%) patients received a gonadotropin releasing hormone (GnRH) analog for 3 months, following the operation. The mean AFS scores at SLL were 15,61±16,41. The difference in AFS scores between the first and SLL was statistically significant ( p
