Özet

Ebstein anomalisi, triküspit kapağın ender bir konjenital malformasyonudur ve sıklıkla atrial septal defekt ile birliktedir. Ölüm sıklıkla kardiyak aritmilerden meydana gelir. Ebstein anomalili bir hasta gebelik ve anestezi sırasında çeşitli hemodinamik bozukluk potansiyeline sahiptir.Bu olgu sunumunda Ebstein anomalisi olan bir gebedeki (36 hafta, 3 gün) sezaryen anestezisinin yönetimi bildirilmektedir. Kan basıncı monitorizasyonu için arter kateterinin yerleştirilmesinden sonra genel anestezi indüksiyonu başlatıldı. İntravenöz yol ile dengeli bir anestezi indüksiyonu yapıldı. Anestezi indüksiyonu 2 mg cisatrakuryum, 350 mg tiyopental ve 0.1 mg fentanil ile, entübasyon 100 mg süksinilkolin ile gerçekleştirildi. İndüksiyonu izleyen operasyon süresince hastanın hemodinamisi stabildi. Cerrahi süresince herhangi bir kardiyak sorun gözlenmedi. Bebeğin 1. dk. Apgar skoru 8. 5. dk. Apgar skoru 10’du. Postoperatif seyir komplikasyonsuz olup, hasta postoperatif 3. günde hastaneden taburcu edildi.

Abstract

Ebstein's anomaly is a rare congenital malformation of the tricuspid valve, often associated with an atrial septal defect. Death occurs usually from cardiac arrhythmias. A patient with Ebstein's anomaly during pregnancy and anesthesia has potential for a variety of hemodynamic disturbances.In this case report, we report the anesthetic management of Cesarean section in a pregnant (36 weeks and 3 days) woman with Ebstein's anomaly. After arterial catheter was inserted for monitoring blood pressure, general anaesthesia was started. Anaesthesia was induced smoothly and rapidly through intravenous route. Anaesthesia induction was performed with cisatracurium 2 mg, tiyopentone 350 mg and fentanyl citrate 0.1 mg and intubation was performed with 100 mg succinylcholine. Following induction, her hemodynamics were stable during perioperative period. No cardiac events were observed during surgery. The baby had Apgar score of 8 at 1 minute and 10 at 5 minute. The postoperative course was uneventful, and the patient was discharged from the hospital on the 3rd postoperative day.This case illustrates the importance of careful attention to the preoperative findings and the perioperative hemodynamic parameters.
