Çok kullanımlık solunum devresinde tek kullanımlık filtrenin trakeal tüp ve laringeal maske ile birlikte kullanılmasının bakteriyel kontaminasyon yönünden güvenilirliği

Özet
Amaç: Çalışmada, çok kullanımlık solunum devrelerinin % 2 gluteraldehit ile temizliğinin etkinliği, bakteriyel üreme oranı ile filtre kullanımının, laringeal maske ve trakeal tüp kullanımının üreme sıklığını nasıl etkilediği araştırılmıştır. Gereç ve Yöntem: Çalışmada, günlük temizliği % 2’lik gluteraldehit solüsyonu ile yapılmış çok kullanımlık solunum devrelerinden örneklem tarzı 198 örnek alınmıştır. Örnekler, devrelerin filtre ile trakeal tüp (Grup FTT), filtre olmaksızın trakeal tüp (Grup TT), filtre ile laringeal maske (Grup FLM), filtre olmaksızın laringeal maske (Grup LM) ile kullanımı ve devrenin strelizasyonu sonrası (Grup SS) devrenin Y-parçası içinden sürüntü tarzı alınıp, buyyonlu transfer besiyerine ekilerek bekletilmeden laboratuvara ulaştırılmışlardır. Örnek alındığında devrenin temizlik sonrası kaç dk. ve kaç hastada kullanıldığı kayıt edilmiştir.Bulgular: Yüz doksan sekiz örnekten 64 tanesinde (% 32,3) üreme saptanmıştır. En yüksek üreme % 85,0 sıklıkta Grup LM’de, en düşük üreme ise % 6,9 sıklıkta Grup SS’de görülmüştür. Filtre ve trakeal tüp kullanımı, filtresiz kullanıma ve laringeal maske kullanımına oranla bakteriyel bulaşmayı azaltmıştır (p=0,009, p=0,015). Devrenin kontamine olma sıklığı, kullanım zamanından bağımsız, kullanıldığı hasta sayısı ile orantılıdır. Özellikle iki hastadan fazla hastada kullanılan devrelerde üreme anlamlı olarak yükselmiştir (p=0.001). Tartışma ve Sonuç: % 2’lik gluteraldehit ile soğuk sterilizasyon işlemi sonrası % 6,9 sıklıkta üreme görülmesi yöntemin çok kullanımlık solunum devrelerinde etkin olmadığını göstermiştir. Her ne kadar tek kullanımlık filtre ve trakeal tüp kullanımı üreme sıklığını azaltsa da, bu çalışmada; çok kullanımlık solunum devresi kullanımının bakteriyel kontaminasyon ve devrelerin bakteriyel vektör olabilmesi yönünden güvenli olmadığı görüşüne varılmıştır.
Abstract
Aim: In this study, the effectiveness of disinfection with 2 % gluteraldehide of reusable ventilator circuits was investigated as well as the evaluation of whether bacterial filter protects against bacterial production with the use of laryngeal mask or endotracheal tube.Materials and Method: Onehundred and ningtyeight samples were taken from the reusable ventilator circuits disinfected daily with 2% gluteraldehide. Samplings were done from connection of filter and tracheal tube (Group FTT), tracheal tube without filter (Group TT), filter and laryngeal mask (Group FLM), laryngeal mask without filter (Group LM) and from the Y part of circuit, after disinfection (Group SS), and implanted into culture medium and transferred to the laboratory immediately. The length of time of use and the number of patients that the circuit was used after its disinfection were also recorded.Results: Bacterial production was observed in 64 samples out of 198 (32.3 %). Rate of bacterial growth was highest in Group LM with a rate of 85,0 % and lowest in Group SS with 6,9 %. Use of filter and tracheal tube decreased contamination compared to the use of without filter and use of laryngeal mask (p=0,009; p=0,015). Rate of contamination was independent of time of use but was correlated with the number of patients the circuit was used in. Especially in the circuits which were used in more than two patients, bacterial contamination was increased (p=0,001).Conclusion: Cold sterilization with 2 % gluteraldehide proved to be ineffective for reusable ventilator circuits as was shown by bacterial growth at a rate of 6,9 %. Although the use of disposable filter and tracheal tube decreased bacterial growth, we propose that reusable circuits seem unsafe in terms of bacterial contamination.
