Pregabalinin aksiller blok sonrası analjeziye etkisi

Özet
Amaç: Bu çalışmada; aksiller blok ile el ve ön kol cerrahisi geçirecek olgularda, ameliyattan 1 saat önce tek doz alınan 150 mg pregabalinin; blok özellikleri ve postoperatif analjezi üzerine etkisinin değerlendirmesi amaçlanmıştır. Yöntemler: Çalışmaya dahil edilen 60 olgu randomize olarak iki gruba ayrıldı; Grup Pye (n=30) cerrahiden 1 saat önce 150 mg pregabalin ve Grup Kya (n=30) plasebo tablet verildi. Rutin monitörizasyon sonrası aksiller blok uygulandı. Tüm olguların solunum ve hemodinamik parametreleri, aksiller blok özellikleri, sedasyon skorları, postoperatif ağrı şiddeti, ilk ağrı başlama ve ilk analjezik gereksinim zamanları kaydedildi. Bulgular: Hemodinamik ve solunum ile ilgili değişkenler, aksiller blok özellikleri, postoperatif ağrı başlama ve ilk analjezik gereksinim süreleri gruplar arası benzer bulunurken; postoperatif dönemde kaydedilen ağrı skorları, 4. ve 6. saatlerde Grup Pde daha düşük bulundu (p=0,002, p=0,002). Ayrıca ilk 24 saatte Grup Kda bir olgunun, Grup Pde ise dokuz olgunun analjezik gereksinimi olmadı (p=0,012). Sonuç: Periferik rejyonal blok ile cerrahi uygulanacak olgularda cerrahiden 1 saat önce tek doz 150 mg pregabalinin; postoperatif ağrıyı ve analjezik gereksinimini azalttığı kanısındayız.
Abstract
Objective: To evaluate the effects of preoperative, single dose 150 mg Pre- gabalin on block characteristics and postoperative analgesia in patients who undergo hand and forehand surgery with axillary block. Methods: Sixty patients were randomly divided into two groups: Group P (n=30) received 150 mg Pregabalin and Group K (n=30) received a pla- cebo capsule 1 hour before the surgery. After routine monitoring, axillary block was performed. Respiratory and haemodynamic parameters, block characteristics, sedation scores, severity of postoperative pain, time of on- set of postoperative pain and analgesic requirement time were recorded. Results: While the haemodynamic parameters, characteristics of axillary block, onset of postoperative pain and analgesic requirement time were similar between the groups, postoperative 4th and 6th hour pain scores were lower in Group P (p=0.002, p=0.002). Also one patient in Group K and nine patients in Group P did not require any analgesics in the first 24 hour (p=0.012). Conclusion: We concluded that a single dose of 150 mg Pregabalin, given one hour before axillary block, reduces postoperative pain and analgesic requirements in the early postoperative period
