Özet

Bu çalışmada kliniğimize başvuran yumuşak doku sarkomlu hastalar prognoza etkili faktörleri belirleyebilmek için retrospektif olarak değerlendirilmiştir. 1990-2000 tarihleri arasında takip edilen 144 hasta mevcuttur. Ortalama yaş 43'tür. Erkek hasta 76, kadın hasta 68'dir. En sık leiomiyosarkom (%20, n=29), 2.sıklıkla malign fibröz histiositom (%15, n=21), 3.sıklıkla liposarkom (%12,n=17) görülmüştür. Ellibeş hasta stage IV (%38), 56 hasta stage III (%39), 23 hasta (%23) stage I-II idi. Genel sağ kalıma etkili faktörler olarak; yaş (p=0.0029), stage (p=0.0000), lenf nodu tutulumu (p=0.0000), radyoterapi uygulanması (p=0.0312) [postoperatif (p=0.0000)], kemoterapi uygulanması (neoadjuvant veya adjuvant (p=0.0000), kemoterapi rejimi olarak MEID uygulanması (p=0.0192), metastaz bulunması (p=0.0001) ve metastazektomi uygulanması (p=0.0269) bulunmuştur. Nükse etkili faktörler olarak; histopatolojik tip(p=0.0499), tümörün derin yerleşimi (p=0.0366) ve lenf nodu tutulumu (p=0.0000) bulunmuştur. Metastaza etkili faktörler olarak; histopatolojik tip (p=0.0496), grade (p=0.0363) ve yerleşim yeri (p=0.0210) tespit edilmiştir.

Abstract

in this study, patients that came to our clinic with soft tissue sarcoma have been investigated to determine prognostic factors retrospectively. One hundred fourty four patients have been followed up between the years 1990-2000. The average age is 43. 76 of the patients are male and 68 are female. The most often seen first is leiomyosarcoma (20%, n=29), 2nd is malignant fibrous histiocytoma (15%, n=21), 3rd is liposarcoma (12%, n=17). Of these patients 55 were stage IV (38%), 56 were stage III (39%) and 23 were stage I (23%). Factors effecting overall survival were found as: age (p=0.0029), stage (p=0,0000), lymph node involvement (p=0.0000), radiotherapy (p=0.0372) [postoperative p=(0.0000)], chemotherapy (neo-adjuvant or adjuvant (p=0.0000), MEID application as chemotherapy regime (p=0.0197), presence of metastasis (p=0.0001) and application of metastasectomy (p=0.0269); factors effecting recurrence were found as: histopathological type (p=0.0499), deeply embedded tumour (p=0.0366) and lymph node involvement (p=0.0000). Factors effecting metastasis were found as: histopathological type (p=0.0496), grade (p=0.0363) and location (p=0.0210).
