Kısa süreli idrar sonrası uygulanan hastalarda bakteriyüri gelişiminin araştırılması

Özet
Bu çalışma Ege Üniversitesi Tıp Fakültesi Kadın Doğum Kliniği'nde çeşitli jinekolojik hastalıkları nedeniyle öpere edilen hastalara kısa süreli idrar sondası uygulaması sonrası bakteriyüri gelişme olasılığını saptamak amacıyla yapılmıştır. Operasyon öncesi bakteriyüri ve lökositüri görülmeyen, yaşları 26 ile 69 arasında değişen 89 hasta çalışmaya alınmıştır. Yaş dağılımı her iki grupta denk olacak şekilde hastalardan 49'una cerrahi amaçlı olarak 48 saat süre ile profilaktik antibiyotik uygulanırken 40 kişiden oluşan diğer hasta grubunda profilaktik antibiyotik kullanılmamıştır. İdrar örnekleri operasyon öncesi gün ile operasyon sonrası 1, 3, 5 ve 7. günlerde olmak üzere toplam beş kez ve kantitatif bakteriyolojik kültür yöntemleri ile incelenmiştir. Profilaktik antibiyotik kullanmayan 40 hastanın yedisinde bakteriyüri ve lökositüri geliştiği saptanmıştır. Bu grupta etken olarak beş hastada Escherichia coli. iki hastada Klebsiella pneumoniae izole edilmiştir. Olgular incelendiğinde yedisinin de ileri yaş grubundan ve genital maligniteli oldukları görülmüştür. Profilaktik antibiyotik kullanan grupta ise ileri yaşta sistoselli bir hastanın idrar kültüründe Pseudomonas aeruainosa üremiştir. İki gruptaki bu sonuçlar istatistiksel olarak anlamlı bulunmuştur. Sonuç olarak, idrar sondası uygulanan hastalarda idrar yolu infeksiyonu profilaksisi için antibiyotik kullanımindikasyonu olmamakla birlikte predispozan faktörlerin bulunması durumunda olguların bakteriyolojik yönden izlenmesi yararlı ve gereklidir.
Abstract
77J/S study was planned to determine the possibility of after short tenm urinary catheterisation in patients who had operation in the Clinic of Obstetrics and Gynecology Ege University Hospital. Who had not had bacteriuria before operation, evaluated. Prophylactic antibiotic was administered to 49 patients for 48 hours before operation, while the rest 40 patients were not given any antibiotic. Urine samples were investigated by quantitative bacteriological culture methods before the day of operation and on the postoperative days of 1, 3, 5 and 7. Bacteriuria and pyuria were detected in seven of 40 patients who had not used, prophyactic antibiotic. In this, Escherichia coli was isolated from fiveand Klebsiella pneumoniae from two patients. It was observed that these seven were from advanced age group and had genital malignancy. Bacteriuria was detected only in one patient with cystocele and at advanced age from the group who had used prophylactic antibiotic and Pseudomonas aeruainosa was isolated from the urine of this patient. Difference between the culture positivity of these two groups was found to be significant. It is concluded that it is necessary to follow up cases who have predisposing factors for urinary infections although urinary catheterisation is not an indication for antibiotic prophylaxis.
