Ganciclovir therapy of neonatal hepatitis due to congenital cytomegalovirus infection- report of two cases

Özet

Konjenital sitomegalovirus (CMV) infeksiyonu multiorgan tutulumuyla seyreden ciddi bir hastalıktır. Sitomegalovirus infeksiyonunun neden olduğu neonatal hepatit siroza ve son dönem karaciğer yetmezliğine gidiş gösterebilir. Tedavide çeşitli ilaçlar denenmektedir. Gansiklovirin konjenital CMV infeksiyonunda kullanımıyla ilgili yeterli deneyim olmamakla beraber bazı olgularda yararlı olduğu da bildirilmektedir. Bu yazıda konjenital CMV infeksiyonuna bağlı neonatal hepatit gelişen ve gansiklovir ile tedavi edilen iki olgu sunulmuştur. Tedavi öncesi ve "sonrası karaciğer biyopsileri yapılmıştır. Tedavi ile klinik, laboratuvar ve patolojik bulgularda iyileşme izlenmiştir. Gansiklovirin konjenital CMV infeksiyonunda umut verici bir ilaç olduğu görüşüne varılmıştır.

Abstract

Congenital cytomegalovirus (CMV) infection is a serious disease with multiorgan involvement. Neonatal hepatitis due to CMV infection may progress to cirrhosis and endstage liver disease. Many drugs have been tried in the treatment. Although experience with ganciclovir in the treatment of congenital CMV is limited, beneficial effects of the drug have been reported in some cases. In this paper, two cases of neonatal hepatitis due to the congenital CMV infection treated with ganciclovir are presented. Liver biopsies were performed before and after the treatment. The clinical, laboratory and pathological findings showed improvement in patients by therapy. It is concluded that ganciclovir is a promising agent in cases of congenital CMV infection.
