Özet
Onyedi aylık kız olgu ateş yüksekliği, sol bacak üst ve ön yüzündeki mor lekeler ve şişlik nedeni ile başvuru üzerine yatırıldı. Öyküsünden altı gün önce suçiçeği tanısı konulduğu öğrenilen olgunun fizik muayenesinde sol bacakta ve gluteal bölgelerde daha çok sayıda ve değişik boyutlarda olmak üzere her iki ekstrimitede purpura, ekimoz ve nekroz alanları vardı. Protein-C, protein S, antitrombin III düzeyleri düşük; antikardiyolipin antikorları yüksek bulunan olgu 'suçiçeği infeksiyonu sonrası gelişen purpura fulminans olarak değerlendirildi. Hastaya antibiyotik tedavisi ile birlikte düşük molekül ağırlıklı heparin ve taze donmuş plazma verildi. İzleminin yedinci gününde d-dimer ve kanama diyatez testleri normale döndü, ancak protein C, protein S, antitrombin III düzeyindeki düşüklük devam etti. Sol bacaktaki nekrozun kemik dokusuna kadar ilerlemiş olması nedeni ile debritman ve flep için İzleminin sekizinci günü plastik cerrahi merkezine gönderildi. Hastalığın iyileşmesini takiben yapılan kontrollerde protein C, protein S, antitrombin III ve antikardiyolipin antikorları normal bulundu. Olgu, su çiçeğinin nadir bir komplikasyonu olması nedeniyle sunuldu.
Abstract
A seventeen-month-old girl was admitted to hospital with the complaints of high fever, swelling in the whole left leg and echimotic lesions on anterior side of the upper part of the left leg. In history she had been diagnosed to have varicella six days before the hospitalization. On physical examination there were many small petechial and purpuric hemorrages and various sized echimotic and necrotic lesions over the skin covering the buttocks and both legs. The lesions were more marked on the left leg. Protein C, protein S and antithrombine III levels were low and anticardiolipin antibodies were high. Thus the diagnosis of varicella associated purpura fulminans was established. Antibiotherapy, fresh frozen plasma and low molecular weight héparine therapies were given. On the 7th day of hospitalization, d-dimer and the tests of haemorhagic diatesis were normal but protein C, protein S and antithrombine III levels were still low. Because the necrotic lesions on the left leg progressed to the osseos tissue, she was referred to the Plastic Surgery Center for debridment and grafting on the 8th day of hospitalization. Protein C, protein S and antithrombine III levels were found normal after the patient improved.
