Hemodiyaliz hastalarında Legionella pneumophila antikorlarının araştırılması

Özet

Bu çalışmanın amacı, hemodiyaliz hastalarında Legionella pneumophila antikor prevalansını saptamak idi. Ege Üniversitesi Tıp Fakültesi ile Akdeniz Üniversitesi Tıp Fakütesi hemodiyaliz hastalarına ait serum örneklerinde, L. pneumophila antikor prevalansı ve hemodiyaliz süreleri incelenmiştir. İndirekt floresan antikor (İFA) yöntemi ile, 122 hemodiyaliz hastası ve 86 kontrol grubu serum örneğinde L pneumophila antikorları araştırılmıştır. Hasta grubunun 23 (%18.9)'ünde, kontrolların ise dördünde (%4.7) pozitiflik saptanmıştır. İki grup arasındaki fark istatistiksel olarak anlamlı bulunmuştur (p= 0.003). Hasta grubu hemodiyaliz süreleri açısından incelenmiş; L pneumophila antikoru saptanan 23 hastanın ortalama hemodiyaliz süresi 40.2 ay; antikor saptanmayan 99 hastanın ise 29.2 ay olup aradaki fark istatistiksel olarak anlamlı bulunmuştur (p = 0.042). Hasta ve kontrol grubunun, yaş ortalamaları ile kadın-erkek cinsiyet oranı karşılaştırıldığında; aralarında istatistiksel olarak anlamlı fark bulunmamıştır. Çalışmada, antikor prevalansı ve hemodiyaliz süresi hasta grubunda kontrol grubuna göre anlamlı bulunmasına karşın, gerek konuyla ilgili yeterli sayıda araştırma yapılmaması gerekse sonuçların çelişkili olması, bu konuda daha fazla çalışmaya gereksinim olduğunu göstermektedir.

Abstract

The purpose of this study was to determine the prevalence of Legionella pneumophila antibodies in hemodialysis patients. Hemodialysis patients in the Ege University Hospital and Akdeniz University Hospital were studied. The prevalance of L pneumophila antibody was examined in the serum species of the 122 patients and control group (86) by indirect fluorescent antibody (IFA) method. Positive results were obtained in 23 patient (18.9%) and 4 (4.7%) of the control group. The difference between these two groups was found to be statistically significant (p= 0.003). The patient group was investigated in respect to the period of hemodialysis. The mean hemodialysis period was 40.2 months in 23 L. pneumophila antibody-positive patients and 29.2 months in 99 antibody-negative patients. The difference between these two groups was found statistically significant (p= 0.042). When the mean age and sex of the test and control groups were compared, there was no statistically significant difference in between. There was no significant difference between the test and control groups as to antibody prevalence and hemodialysis period. It is concluded that further studies on the subject are required for there are only limited number relevant studies some of which present contradictory results.
