Hepatosellüler karsinomalı hastalarda transfusion transmitted virus (TTV) infeksiyonu prevalansı

Özet

Hepatosellüler karsinoma (hepatocellular carcinoma; HCC) gelişmesinde, özellikle hepatit B virüs (HBV) ve hepatit C virüs (HCV)'un rolü olduğu bilinmektedir. Ancak hepatosellüler karsinomah hastaların küçük bir yüzdesinde etyoloji açıklanamamıştır. Bu çalışma, yeni keşfedilen bir DNA virüsü olan transfusion transmitted virus (TTV)'un hepatosellüler karsinoma ile ilişkisini araştırmak için planlanmıştır. Çalışmaya 14'ü HBV, sekizi HCV ile infekte, üçü alkole sekonder ve ikisi kriptojenik olmak üzere toplam 27 HCC'h hasta ve kontrol grubu olarak belirgin bir karaciğer patolojisi bulunmayan 16 kişi alındı. Transfusion transmitted virus DNA'sının karaciğer biyopsi örneklerinde saptanması için Okamoto'nun önerdiği primerlerle yapılan semi-nested polimeraz zincir reaksiyon (PCR) testi kullanıldı. Hepatosellüler karsinomah hastaların beşinde (%18.5) ve kontrol grubunun ikisinde (%12.5) TTV DNA saptandı (p>0.05). Hasta ve kontrol gruplarındaki TTV prevalansları arasında önemli farklılık bulunmadı. Transfusion transmitted virus ile infekte olan ve olmayan karsinomah hastalarda biyokimyasal ve klinik bulgular benzerdi. Sonuç olarak, bu çalışmanın bulguları TTV infeksiyonu ile HCC arasında bir ilişkinin olmadığını göstermektedir.

Abstract

The role of hepatit B virus (HBV) and hepatit C virus (HCV) in the development of hepatocellular carcinoma (HCC) is known. However, etiology in small proportion of patients with HCC has not been clarified. This study was planned to investigate the relationship between TTV and HCC. A total of 27 patients with HCC (14 infected with HBV, 8 with HCV, 3 with alcoholic hepatitis and 2 with criptogenic hepatitis) and as a control group 16 subjects who had no apparent liver pathology were included in the study. Semi-nested polymerase chain reaction (PCR) test with the primers described by Okamoto was performed to detect TTV DNA in liver biopsy specimens. Transfusion transmitted virüs DNA was detected in 5 (18.5%) of patients with HCC and 2 (12.5%) of the control group. There was no significant difference as to prevalence of TTV between the patient and control groups (p>0.05). Biochemical-parameters and clinical findings were similar in TTV DNA positive and negative patients. In conclusion, the results of this study suggest that there is no association between TTV infection and HCC.
