Özet

Stafilokoklarda makrolitler, linkozamitler ve streptogramin B grubu antibiyotiklere karşı direnç gelişimi en sık, ribozomlarda bulunan ve bu ilaçların bağlanarak etki gösterdikleri hedef moleküllerde değişiklik sonucu ortaya çıkmaktadır. Makrolit-Linkozamit-Strepto-gramin B (MLSB) direnci, yapısal (MLSBc) veya indüklenebilir (MLSBi) özellikte olabilmektedir. Çalışmanın amacı, Staphylococcus aureus suşlarında indüklenebilir klindamisin direncinin prevalansını araştırmaktır. Yazarların hastanesinde 2002-2004 yıllarında izole edilen 275'i metisiline dirençli 526 S. aureus susunda MLSBi direnci D-test yöntemi ile araştırıldı. Suşlar Muelter-Hinton ağara inoküle edildi ve 15 mm mesafe olacak şekilde klindamisin (CL) ve eritromisin (ER) diskleri yerleştirildi, inkübasyon sonrasında CL diski etrafında "D" şeklinde inhibisyon zonu gözlenen suşlar D-test pozitif kabul edildi, indüklenebilir CL direnci 275 MRSA susunun 102'sinde (%37) pozitif saptanırken, 251 metisiline duyarlı S. aureus susunun altısında (%2.4) pozitif idi. Metisiline dirençli S. aureus kökenlerinde bulunan yüksek MLSBi direnci, rutin antibiyogramda CL'ye duyarlı görünen suşların aslında dirençli olabileceklerini göstermiştir. Antibiyotik duyarlıhk sonuçları verilirken bu durum dikkate alınmalı ve ER'ye dirençli, CL'ye duyarlı tüm S. aureus suş¬larında D-test yapılmalıdır.

Abstract

Ribosomal target modification is the most common mechanism of acquired resistance to macrolides, lincosamides and strepto¬gramin B antibiotics in staphylococci. Macrolide-Lincosamide-Streptogramin B (MLSB) resistance can be either constitutive (MLSBc) or inducible (MLSBi). The aim of this study was to determine the prevalence of inducible clindamycin resistance among staphylo¬cocci in a university hospital. Two hundred and seventy-five methicillin-resistant Staphylococcus aureus (MRSA) and 251 methicillin-sensitive S. aureus (MSSA) were tested for inducible resistance by using D-test. Clindamycin (CL) and erythromycin (ER) disks were placed 15 mm apart from center to center on Mueller-Hinton agar plates and the D-shaped inhibition zones seen on the plates were interpreted as D-test positive. Inducible clindamycin resistance was detected in 102 (37%) of 275 MRSA and in 6 (2.4%) of 251 MSSA isolates. The high rate of MLSBi resistance among the MRSA isolates points out that an important number of strains found to be CL sensitive by standard disk diffusion method are resistant to CL and therapeutic failures during treatment with CL is to be expected. Antimicrobial susceptibility results should be a guide in the treatment of such infections to prevent therapeutic failures. D-test should be performed for all S. aureus isolates which are found to be ER- resistant but CL sensitive.
