Üst gastrointestinal sistem kanamalarında helikobakter pylori enfeksiyonunun sıklığı ve önemi

Özet
Amaç: Ülkemizde en sık üst gastrointestinal sistem (GIS) kanama nedeni peptik ülser olup, helikobakter pylori (H. pylori) enfeksiyonu ve non-steroidal antiinflamatuar ilaç (NSA‹‹) kullanımı peptik ülser gelişiminde başta gelen etmenlerdir. Bu çalışmadaki amacımız üst GIS kanaması ile gelen hastalarda H. pylori enfeksiyonunun sıklığını ve önemini araştırmaktır. Materyal ve Metod: Acil Dahiliye Kliniğine hematemez ve/veya melana şikayeti ile gelen, yapılan üst GIS endoskopilerinde kanama odağı saptanan ve H.pylori varlığını araştırmak için biyopsi örneği alınan 102 erişkin hasta çalışmaya alındı. Hastaların demografik bilgileri, son 10 gün içinde NSA‹‹ kullanım öyküsü, üst GIS endoskopi sonuçları kaydedildi. H. Pylori enfeksiyonu varlığı histopatolojik yöntemle araştırıldı. istatistiksel analizde SPSS 12.0 paket programı kullanıldı.Bulgular: Hastalar 18 ile 90 yaş aralığında idi (Yaş ortalaması: 54±18 yıl). Vakaların 66 tanesi erkek (%64.7), 36 tanesi kadın (%34.3) idi. Endoskopik işlem sonrasında 69 hastada duodenal ülser (%67.6), 18 hastada gastrik ülser (%17.6), 13 hastada eroziv gastroduodenit (%12.7), 1 hastada özefajit (%0.8), 1 hastada Mallory-Weiss sendromu (%0.8) saptandı. Hastaların %62.7’sinde H. pylori enfeksiyonu saptandı. Duodenal ülserli hastaların %69.6’unda, gastrik ülserli hastaların% 55.6’sında, eroziv gastroduodenitli hastaların % 38.5’inde H. pylori pozitif bulundu. Hastaların % 65.3’ünde NSAii kullanımı söz konusuydu; %40.6’sında H. pylori enfeksiyonu ve NSAi i kullanımı birlikteliği saptandı. Hastaların ancak %11.9’unda H. pylori ve/veya NSA‹‹ kullanımı sözkonusu değildi.Sonuç: Bu çalışma üst GIS kanamalı hastalarda peptik ülser prevalansının azımsanmayacak kadar fazla olduğunu ortaya koymaktadır. Üst GIS kanaması ile gelen hastalarda antrumdan biyopsi alarak veya noninvaziv yöntemlerle H.pylori varlığının araştırılması ve eradikasyon tedavisinin ihmal edilmemesi gerekmektedir.
Abstract
Objective: Peptic ulcer is the most common cause of upper gastrointestinal (GIS) bleeding in our country. Helicobacter pylori (H. pylori) infection and nonsteroidal anti-inflammatory drug (NSAID) use are the leading factors in the development of peptic ulcer. In this study we aim to investigate the prevalence and importance of H.pylori infection in patients with upper GIS bleeding. Material and Method: A hundred and two adults who applied to Emergency Service with hematemesis and/or melana complaint, having bleeding focus detected by upper GIS endoscopy and biopsed to search for H.pylori, were included in the study. Demographic background, NSAID use within last 10 days, and upperGIS endoscopy results of the patients were recorded. Presence of H.pylori infection was investigated by histopathological method. SPSS 12.0 package was used for statistical analysis. Findings: The patients were between 18 and 90 years of age (Mean: 54±18). Sixty six of the cases were male (64.7%), 36 were female (34.3%). After endoscopic procedure 69 patients had duodenal ulcer (67.6%), 18 had gastric ulcer (17.6%), 13 had erosive gastroduodenitis (12.7%), 1 had esophagitis (0.8%), and 1 had Mal-infection was detected in 62.7% of the patients. H.pylori was positive in 69.6% of the patients with duodenal ulcer, 55.6% of the patients with gastric ulcer and 38.5% of the patients with erosive gastroduodenitis. 65.3% of the patients had used NSAID. Only 11.9% of patients did not have H.pylori or use NSAID. Result: This study showed the prevalence of peptic ulcer in patients with upper GIS bleeding was substantially high. It is important to investigate the presence of H.pylori by antral biopsy or noninvasive methods and not to disregard eradication treatment.
