Özet
Giriş: Appendektomi, gebelikte jinekolojik prosedürler dışında en sık işlenen cerrahi işlemdir. Bu çalışmada akut apandisit ön tanısı ile opere edilen hastalar retro spektif olarak incelenmiştir. Materyal-Metod: Bu çalışmada Eylül 2004-Nisan 2008 tarihleri arasında Kars Devlet hastanesinde akut appendisit ön tanısı ile opere edilen 22 hasta çalışmaya dahil edilmişir. Sonuçlar: Akut apandisit ön tanısı ile 22 hasta opere edildi. Hastaların yaş ortalaması 26.7 (19-38)yıl idi.Hastaların 5’ i (%22.7 ) I.trimestr, 16 ‘sı(%72.8) II.trimestr, 1‘i (%4.5) III trimestr idi.Hastaların 14’ünde akut apandisit, 5’ünde perfore apandisit, 4 hastada ise appendiks normal bulundu. Fetüs kaybı bir olguda, abortus iki olguda gözlendi. Perfore apandisit olan bir hastada yara infeksiyonu gelişti ve hasta konservatif olarak tedavi edildi. ‹ki olguda ise erken ileus tablosu gelişti ve hastalar konservatif olarak tedavi edildi. Tartışma: Gebelikte oluşan fizyolojik değişiklikler nedeniyle akut apandisit tanısı zordur ve tanıda gecikme maternal ve fetal komplikasyonlara neden olmaktadır. Bu nedenle akut apandisit düşünülen ol- gular dikkatli değerlendirilmeli ve gereksiz appendektomiden kaçınmak için yardımcı tanı yöntemleri kullanılmalıdır.
Abstract
Introduction: Appendectomy is the most achieved surgical procedure in pregnants without gynecologic interventions. In this study, patients operated for possible acute appendicitis, evaluated retrospectively. Material-Metods: In this study, we evaluated 22 pregnant women that operated for acute appendicitis in Kars State Hospital from September 2004 to April 2008. Results: 22 pregnant women were operated with a diagnosis of acute appendicitis. Average age of patients was 26.7 (19-38) years. Five(22.7 % ) patients were at first trimestr, 16 (72.8 %) were at second trimestr and one (4.5 %) at third trimestr. 14 of patients were found as acute appendicitis, five as perforated appendicitis and four as normal. Fetal loss occured in one patient and abortous in two patients. In one patient with perforated appendicitis, wound infection developed and treated as conservatively. Early ileus occured in two patients and they were followed as conservatively. CONCLUSION Diagnosis of acute appendicitis is difficult in pregnancy for the reason of physiologic changes and any delay cause increased maternal and fetal complications. So that cases whit possible diagnosis of acuteappendicitis must be carefully evaluated and assisted diagnostic methods should be used for avoiding unnecessary appendectomy.
