Yara infeksiyonu profilaksisinde sistemik ve topikal kombine sefazolinin sistemik sefazolin uygulamasıyla karşılaştırılması

Sistemik profilaksiyle sistemik ve topikal kombine uygulamanın profilaktik etkinlik üzerindeki etkisini karşılaştırmak üzere temiz ve temiz-kontamine ameliyatlar yapılan 133 hasta yara infeksiyonu profllaksisi için randomize olarak sistemik (Grup I, n=71) ve kombine sistemik ve topikal (Grup II, n=62) sefazolin uygulanan 2 gruba ayrıldı. Yara infeksiyonu için risk faktörleri obezite, ileri yaş (>70 yaş), diabet, malin hastalık, abdominal ve acil ameliyatlar olarak belirlendi. Sistemik ve kombine sistemik ve topikal profilaksi grupları arasında yara komplikasyonlarının insidensi, risk faktörleri ve ameliyat tiplerinin dağılımı açısından istatiksel anlamlı fark yoktu. Grup l'de 2 hastada (% 2.8), Grup H'de J hastada (% 1.6) yara infeksiyonu gözlendi. Gruplar arası fark istatistiksel olarak anlamlı değildi. Sonuç olarak sefazolinin sistemik ve topikal kombine kullanımının yara infeksiyonu profilaksisinde ek bir etkisinin olmadığı görüldü.
Abstract
This study was designed to compare the effectiveness of systemic and combined use of topical and systemic antibiotic prophylaxis for surgical wound infection. One hundred thirty-three patients undergoing clean and clean-contamined operations were randomized to receive systemic (Group I, n=71), or combined systemic and topical (Group II, n=62) cefazolin prophylaxis for surgical wound infection. Risk factor for wound complications (obesity, age greater than 70 years, diabetes, malignancy, abdominal and emergency operations) were recorded. The incidence of wound complications, risk factors and types of operations were compared between the systemic and combined systemic and topical prophylaxis groups, and no statistically significant differences were found. Wound infection was observed in two patients in Group I (2.8 %), and in one patient in Group II (1.6 %). The difference two groups was not statistically significant. As a result combined use of systemic and topical cefazolin was not showed an additive effect in wound infection prophylaxis.
