Farelerde oluşturulan osteomyelit modellerinde yabancı cisim uygulanmasının lokal ve sistemik infeksiyon bulguları üzerine etkileri

Özet
Giriş: Bu çalışmada farelerde oluşturulan akut hematojen ve post-travmatik osteomyelit modellerinde, kemiğe yerleştirilen yabancı cisimlerin lokal infeksiyon bulguları ve diğer organ tutulumları üzerine etkileri araştırılmıştır. Gereç ve Yöntem: Çalışmada kullanılan 30 adet fındık faresi beş gruba ayrıldı. Bütün deneklerde medüller kanal hasarı oluşturulup ilk grup kontrol grubu olarak alındı. İkinci ve üçüncü grupta metisiline dirençli Staphyhcoccus aureus (MRSA) susu ile hematojen osteomyelit modeli oluşturudu ve 3. grupta medüller kanala silikon dren parçası konuldu. Dördüncü ve beşinci grupta MRSA ile post-travmatik osteomyelit modeli oluşturularak 5. grupta medüller kanala silikon dren parçası konuldu. Fareler 10. günde öldürülerek tibiaları makroskopik, mikrobiyolojik ve histopatolojik olarak incelendi. Kan kültürü örneklerinin ekimi yapıldı. Karaciğer, dalak, böbrek, kalp kası, akciğer ve beyin dokularından alınan örnekler histopatolojik olarak incelendi. Grup 2 ile 3'ün bulguları ve Grup 4 ile 5'in bulguları SPSS 9.05 istatistik programında relatif risk tahmini hesaplanarak karşılaştırıldı. Bulgular: Kontrol grubunda lokal ve sistemik infeksiyon bulgusuna rastlanmadı. Hematojen ve post-travmatik osteomyelit modellerinde yabancı cisim uygulanmasının lokal ve sistemik infeksiyon bulguları üzerine bir etkisi olmadığı gözlendi. Tartışma: Osteomyelitte, bakterinin kemiğe yerleşimi sürecinin ortamda bulunan yabancı cisimlerden bağımsız olarak seyrettiği kanısına varıldı.
Abstract
Introduction: In this study, by inducing acute hematogenous and post-traumatic osteomyelitis in mice, the effect of the foreign body fitted into bone on local infection findings and other organ involvement were investigated. Materials and Method: Thirty mice used in this study were separated into five groups. In all of the subjects the medullary canals were destroyed and the first group formed the control. In the 2nd and the 3rd groups hematogenous osteomyelitis were developed using meticilline resistant Staphy/ococcus aureus, and in the 3rd group a piece of silicon drain tube was placed into the medullary canal. In the 4th and the 5th groups post-traumatic osteomyelitis were induced using meticilline resistant Staphy/ococcus aureus and in the 5th group a piece of silicon drain tube was placed into the medullary canal. Ten days later mice were killed and tibias were studied by macroscopically, microbiologically, and histopathologically. Blood samples were inoculated in agar plate. Samples taken from the liver, the spleen, the kidney, the heart muscle, the lung, and the brain tissues were observed histopathologically. Findings of the group 2 versus 3 and the group 4 versus 5 were statistically compared by using SPSS 9.05 program.Results: There was no local or systemic finding of infection in the control group. The application of foreign body in hematogenous and post-traumatic osteomyelitis models did not influence local or systemic infection findings. Conclusion: Our findings suggest that, the process of bacteria settlement in bone is independent from foreign bodies present in the environment.
