Özet

Amaç: Bu çalışmada SOF'ün mikroanatomik yapısı cerrahi yaklaşımda sinir ve damar yapılarının ortaya konulabilmesine yönelik olarak değerlendirildi. Gereç ve yöntem: Çalışmada 5 adet kurukafa ve 5 adet kadavra kullanıldı. Kurukafalarda superior orbital fissürün boyu ve eni ölçüldü. Formalinde fikse edilmiş kadavraların a. karotis kommunis, a. vertebralis ve v. jugularis internaları cateterize edildi. Damarlar renkli silikon ile dolduruldu. Disseksiyon safhaları fotoğraf kamerası ile görüntülendi. Sonuçlar. Orbita tavanında ve orbita lateral duvarında SOF'ün ortalama 1,5 cm supero-medialine ve 1,5 cm infero-lateraline kadar uzanan kemik rezeksiyonunun SOF'ün srtaya konulabilmesi için yeterli olduğu saptandı. Periorbita açıldıktan sonra frontal sinirin önemli bir işaret noktası oluşturduğu gözlendi. SOF içinde yer alan superior oftalmik venin karotiko-kavernöz fistüllerin tedavisinde cavernöz sinüse ulaşmada alternatif bir yolu oluşturduğu izlendi.

Abstract

Objective: In this study, microanatomy of SOF was evaluated in the scope of surgical exposure feasibilities of the nerves and the vascular structures. Materials and methods :Five skulls and 5 cadaveric heads were used in this study. The length and the diameter of the superior orbital fissure were measured in the skulls. Arteria carotis communis, arteria vertebralis, and vena jugularis interna were catheterized in the formalin fixed cadavers. The vessels were injected with colored silicone. The stages of the dissection through a photographic camera. Conclusions: Bone resection on roof and lateral wall of the orbit extending 1.5 cm supero-medial and 1.5 cm inferolateral to SOF was found to be sufficient to expose SOF. It was noticed that frontal nerve posed a significant landmark right after the periorbit has been opened. Superior ophthalmic vein passing through was noted to be an alternative route to reach cavernous sinus in the treatment of carotico-cavernous fistula.
