Ciddi Amitriptilin Zehirlenmesi Olan Hastada Hemodiyafiltrasyon Uygulaması

Özet

Trisiklik antidepresan ilaçlar yüksek dozlarda alındıklarında ciddi mortalite ve morbiditeye sebep olmalarına rağmen depresyon tedavisinde yaygın olarak kullanılmaktadırlar. Ciddi kalp ritim bozukluklarına, hipotansiyona ve merkezi sinir sistemi depresyonuna neden olabilirler. Gastrik lavaj ve aktif kömür uygulaması, benzodiazepinler, volüm replasmanı, lidokain ve sodyum bikarbonat infüzyonu gibi konvansiyonel tedavi yöntemlerine rağmen bazı hastalar tedaviye yanıtsız kalabilmektedirler. Bu yazıda Glasgow Koma Skalası skoru 4 ve ciddi aritmileri olan 20 yaşında ciddi amitriptilin zehirlenmesi gelişen hastanın hemodiyafiltrasyon ile tedavi edilmesi sunulmaktadır. Bu hastada konvansiyonel tedavi yetersiz kalmıştır ve hastaya 36 saat boyunca hemodiyafiltrasyon uygulanmıştır. Hastanın 36 saat sonunda bilinci açılmış ve hasta hemodinamik açıdan stabil hale gelmiştir. Bunun gibi konvansiyonel tedaviye cevap vermeyen ciddi zehirlenme olgularında hemodiyaliz, plazmaferez, hemoperfüzyon ve hemodiyafiltrasyon gibi yaklaşımlar kurtarıcı tedavi olabilirler ve göz önünde bulundurulmalıdırlar.

Abstract

Tricyclic antidepressants are widely used drugs for treatment of depression despiteserious mortality and morbidity in higher doses. They can cause severe cardiac arrhythmias,hypotension and central nervous system depression. Some patients may remain unresponsivein spite of conventional therapies such as gastric lavage and activated charcoal administration,benzodiazepines, volume replacement, lidocaine and sodium bicarbonate infusion. In this paper wereport a 20-year-old female patient who had severe amitriptyline intoxication with a Glasgow ComaScale score 4 and severe cardiac arrhythmias and was treated with hemodiafiltration. Conventionaltherapy was inadequate and hemodiafiltration was applied to the patient for 36 hours. At the end ofthe 36-hours the patient was conscious and became stabilized in terms of hemodynamics. In caseof serious intoxication that does not respond to conventional therapy, approaches like hemodialysis,plasmapheresis, hemoperfusion and hemodiafiltration can be a rescue therapy and they should beconsidered.
